摘要
目的探讨妊娠滋养细胞疾病的MRI诊断及病理对照,为该疾病的诊断和治疗提供更好的依据。方法对本院近2年来收治并经手术病理证实的60例妊娠滋养细胞疾病患者的临床资料进行回顾性分析。患者仰卧位,采用腹部线圈进行扫描。先对患者进行T1WI和T2WI常规扫描,矩阵:256×256,层厚:0.6cm,层距:0.1cm。动态增强扫描采用Gd-DTPA造影剂,T1WI横轴面以及矢状面扫描,0.1mmol/kg,肘静脉注射。结果 60例妊娠滋养细胞疾病患者中,葡萄胎26例,平扫可观察到子宫体积增大,宫腔内信号均匀长T1,长T2,囊泡扫描图像呈葡萄状,包膜完整且没有侵犯肌层。囊泡间能观察到条状低信号区。侵袭性葡萄胎患者17例,其他17例。结论妊娠滋养细胞疾病的MRI诊断及病理对照对该疾病的诊断和预防具有积极的指导作用。核磁共振能够很好的显示出病变部位的结构和状况,配合常规的病理诊断,可以有效的提高临床诊断率。
Objective To study the control of MRI diagnosis and pathology of gestational trophoblastic disease, provide a better basis for the diagnosis and treatment of the disease. Methods our hospital in recent 2 years were colleeted and the clinical data of 60 patients with gestational trophoblastic disease confirmed by operation and pathology were analyzed retrospectively.With the patient supine, were scanned with abdominal coil. The first T1WI and routine T2WI scan, the patients: 256 × 256,thickness:0.6 cm, layer spacing:0.1 cm.Dynamic enhanced scan using Gd-DTPA contrast agents, T1WI axial and sagittal scan,0.1 mmol/kg,intravenous injection. Results 60 cases of patients with gestational trophoblastic disease,26 cases of hydatidiform mole,plain scan can observe uterine volume increases,intrauterine signal even long T1,long T2,vesicle scanning image a grape-like,complete capsule and no muscular invasion.Vesicles can be observed a low signal area. 17 cases of patients with invasive hydatidiform mole,17 other cases. Conclusion MRI diagnosis and pathology of gestational trophoblastic disease control has positive guidance for the diagnosis and prevention of the disease. Magnetic resonance imaging can show the structure and status of the lesion is very good,with the pathological diagnosis of routine,can effectively improve the accuracy of clinical diagnosis.
