摘要 目的:通过计算两种用药方式主要麻醉药的用量,核算出经济成本,为指导腹腔镜胆囊切除术(LC)的麻醉用药,降低患者药物费用,提高医疗技术含金量做基础。方法:选择48例美国麻醉师协会(ASA)Ⅰ~Ⅱ级的择期LC患者,随机分为两组,一组麻醉维持异丙酚伍用瑞芬太尼(Pro组)(n=24),一组麻醉维持吸入七氟醚(Sev组)(n=24),两组诱导用药相同,Pro组为异丙酚伍用瑞芬太尼,Sev组为吸入七氟醚,手术终了计算主要麻醉药消耗量并核算成所需费用。结果:两组麻醉后恶心呕吐等并发症比较差异无统计学意义(P>0.05),与Pro组比较,Sev组的维库溴铵用量差异有统计学意义(P<0.05),Sev组所需药物费用较少,差异有统计学意义(P<0.01)。结论:通过两种代表性的用药方式计算出耗药量并核算费用,认为吸入七氟醚组节省费用,值得关注。 Objective:In order to instruct the dosage of anesthetics for laparoscopic cholecystectomy (LC),de- crease the charge of anesthetics and improve the medical technique ,the dosage and the cost of two kinds of anesthetics approaches was measured. Methods:Forty-eight American Society of Appraisers (ASA) I or ~ LC cases were ran- domly divided into two groups(n= 24 each). After using the same induced anesthetics for these two groups,group I propofol remifentanil(Pro group)and group I sevoflurane(Sev group). The doeage and cost of the anesthetics used during the surgery were calculated. Results :Two groups of complications such as nausea and vomiting after surgery no difference (P〈0.05). The dosage of vecuronium for Sev group was less than that of Pro group,showing significant difference (P〈0.05). In addition,the cost of the Sev group was lower than that of Pro group with significant differ- ence (P〈0.01). Conclusion:The cost of the anesthetics can be influenced by many factors. After comparing the cost and dosage of anesthetics at some condition,it is suggested that inhaled sevoflurane has more advantages than propo- fol-remifentanil,and deserves more concerns.
