心电图SavR诊断高血压左心室肥大(LVH)的临床价值 被引量:3

摘要 目的探讨心电图SavR电压在诊断高血压左心室肥大(LVH)中的临床价值。方法经超声心动图检查诊断为左心室肥大的高血压患者75例为观察组及健康体检无左心室肥大的正常人120例为对照组,测量各相关R波、S波电压,计算各肢体导联及胸导联Rv5+Sv1并比较SavR、Rv5+Sv1及两者联用诊断左心室肥大的敏感性、特异性及准确性。结果各肢体导联中SavR敏感性(45.3%)较高(P<0.05),特异性及准确性相近。胸导联Rv5+Sv1敏感性(60.0%)较SavR高,但特异性(85.8%)较SavR(98.3%)低,SavR、Rv5+Sv1准确性为77.9%、75.8%。两者联用时诊断左心室肥大的敏感性和准确性显著提高(分别为88.0%、85.6%)。结论 SavR标准是诊断左心室肥大较好指标,与Rv5+Sv1联用可提高高血压左心室肥大诊断的敏感性和准确性。 Objective To investigate the clinical value of ECG SavR voltage in the diagnosis of hypertensive left ven- tricular hypertrophy (LVH). Methods Seventy-five hypertension patients diagnosed with LVH by untrasounic cardiogram were selected as the observation group and 120 normal people without LVH were selected as the control group. Rele- vant R-wave and S-wave voltage was measured and Rv5+Svl of each limb leads and chest leads were calculated. The SavR, Rv5+Svl and the sensitivity, specificity and accuracy of combined application were compared. Results Of each limb leads, the sensitivity of SavR (45.3%) was high (P 〈 0.05) and the specificity and accuracy were similar. The sen- sitivity of chest leads Rv5+Svl was 60.0%, which was higher than SavR, but the specificity (85.8%) was lower than that of SavR (98.3%); The accuracy of SavR and Rv5+Svl were 77.9% and 75.8%. In the diagnosis of LVH, the sensitivity and accuracy of combined application improved significantly (88.0% and 85.6% respectively). Conclusion SavR is a good indicator in the diagnosis of LVH and its combination with Rv5+Svl can improve the sensitivity and accuracy of the diagnosis of hypertensive LVH.
