延迟腓肠神经营养皮瓣修复小腿和足大面积软组织缺损 被引量:4

摘要 目的探讨延迟腓肠神经营养皮瓣修复小腿中下段、足大面积软组织缺损的临床效果。方法对48例小腿中下段、足大面积软组织缺损(缺损面积7.0 cm×21.0 cm~17.0 cm×35.0 cm)患者采用延迟腓肠神经营养皮瓣修复治疗,供区游离植皮。结果切取皮瓣面积8.0 cm×22.0 cm~18.0 cm×36.0 cm。患者均获随访,时间6~36个月。2例出现表皮坏死,经治疗后愈合;46例均一期愈合,皮瓣质地优良,外观满意,功能良好。结论延迟腓肠神经营养皮瓣有可靠的血供,满意的疗效,在修复小腿、足大面积软组织缺损中是一种实用、经济的方法。 Objective To explore clinical effect of repair large area defects of the lower leg and foot with soft tissue.Methods 48 patients with large area defects of the lower leg and foot were repaired with delayed sural neurovascular flaps.Of the 48 patients,complete debridement was done firstly,then the area of soft tissue was measured.The defects were 7.0cm ×21.0 cm~17.0 cm ×35.0 cm.Island flap that could be transferred was designed on back of the calf.The flap was anatomized and dissociated,completely hemostasised,reserved proximal,middle and distal perforating branch of sural neurocutaneous artery.Results The cut flap area was 8.0 cm×22.0 cm^18.0 cm×36.0 cm.All cases were followed up from 6 months to 36months.Among the 48 cases,46 flaps survived at the first intention,two had skin necrosis on the distal of the flap,which were cured at the second intention.All showed that the wound were covered well the appearance and function were satisfied in cases.Conclusions Delayed neurovascular sural flaps had reliable blood supply and satisfied efficacy,which was an useful and economic method for repairing large area defects of soft tissue on the lower leg and foot.
