128层螺旋CT CARE Dose 4D肺动脉成像的剂量降低及辐射优化 被引量:6

摘要 目的探讨128层螺旋CT应用管电流调制(CARE Dose 4D)技术,在不同条件下(80 kV与100 kV),对肺动脉成像(CTPA)中的影像质量及辐射剂量的影响。方法将65例临床疑似肺栓塞受检者随机分做2组,分别以80 kV(80 kV组,32例)和100 kV(100 kV组,33例)管电压,均使用128层CT及管电流调制技术行肺动脉成像。测量肺动脉主干至双侧亚段肺动脉的CT值,以肺动脉主干CT值的标准差代表图像噪声。记录实际容积CT剂量指数(CTDIvol)、扫描长度和剂量长度乘积(DLP)、并计录有效剂量(ED),对两组指标进行统计学t检验。使用4分法对图像质量进行主观评分,行U检验。结果 80 kV组所测肺动脉CT值均高于100 kV组(P<0.05)。80 kV组CT-DIvol、DLP和ED均低于100 kV组(P值均<0.001)。两组主观评分与图像噪声的差异均无统计学意义(P>0.05)。结论 128层CT管电流调制CTPA 80 kV比100 kV更具优势,ED值更低,图像噪声和主观评分无显著差异。
