摘要
目的分析脓毒症大鼠肝脏缺氧诱导因子-1α(HIF-1α)水平的表达、血清乳酸水平、转氨酶水平及肝脏病理损伤的变化;探讨HIF-1α与乳酸、肝脏转氨酶及病理损伤是否存在相关性。方法将清洁sD大鼠150只分为正常组50只,假手术组50只,盲肠结扎术(CLP)手术组50只。CLP组于手术成功后的第6、12、24、48及72小时分别取10只处死,并于每个时间点取10只正常组大鼠和10只假手术组大鼠作为相应时间点的对照。在各时间点取肝脏组织进行病理学分析,测定血HIF-1α、乳酸及转氨酶水平。比较各时间点血乳酸、转氨酶及HIF-1α水平的表达是否存在差异,并探讨HIF-1α与肝脏损伤指标及病理变化间的联系。结果CLP组大鼠肝细胞6h组开始出现损伤,12h、24h和48h损伤逐渐加重,损伤在CLP后48h最重;CLP组6h、12h和24h HIF-1α水平的表达与正常组和假手术组相比明显增高,差异有统计学意义(P〈0.05);CLP12h和24h乳酸、AIJT及AST水平均明显升高,与正常组和假手术组比较差异均有统计学意义(P〈0.05);HIF-1α水平与乳酸水平、血清ALT水平均存在正相关性(r值分别为0.631、0.436,P〈0.05)。结论脓毒症大鼠血HIF-1α与乳酸及谷丙转氨酶水平存在相关性,与肝脏病理损伤时间基本相同,HIF-1α可作为预测脓毒症肝脏损伤的主要指标。
Objective To study the expression of liver hypoxia inducible factor - 1 α,lactic acid level,transaminase level and liver pathological damage in sepsis rats, and analyse whether it exists a correlation between HIF - 1 α and lactic acid, liver, transaminase and pathological damage. Methods 150 clean SD rats are divided into normal group 50, sham operated group 50 and the cecal ligation and puncture (CLP)50. 10 rats are killed respectively after successful surgery at 6, 12, 24, 48, 72 h in CLP group. At each time point 10 normal rats and 10 sham -operated rats are taken as a control in the corresponding time points, getting liver tissue for pathological analysis, detecting the level of HIF - 1 α, lactate levels and liver transaminase levels. Compare lactate levels, transaminase levels and the expression of HIF - 1α at each time points whether there is a difference, and to explore the link between HIF - 1α and liver damage indicators, pathological changes. Results The rat hepatocytes in CLP group begin appearing at 6 h; at the 12 h, 24 h and 48 h the damage is gradually increasing, the damage at 48 h is heaviest; the expression of HIF - 1α protein in CLP group at 6 h, 12 h and 24 h are significantly higher than the normal group and sham operation group, there is statistically significant difference( P 〈 0.05 ) ; the lactate levels, ALT levels and AST levels in CLP group at 12 h and 24 h are significantly elevated compared with the normal group and sham operation group , which the differences are statistically significant ( P 〈 0. 05 ) ; Comparing HIF -1α levels with lactate levels and serum ALT levels, there are positive correlation between them (r = 0. 631, 0. 436, P 〈 0.05 ). Conclusion In sepsis the level of HIF - 1α is correlated with lactate levels and ALT levels, and it is the same time with liver pathological damage. So the HIF -1α can be used as a key indicator to predict liver damage in seosis rats.
