摘要
目的比较难治性抑郁症与非难治性抑郁症患者自动思维的差异，以及抑郁、焦虑症状对自动思维的影响。方法采用自动思维问卷（ATQ）、汉密尔顿抑郁17项量表（HAMD）、汉密尔顿焦虑量表（HAMA）对入组抑郁症患者进行测查。结果①难治性抑郁症与非难治性抑郁症患者自动思维[（81．57±21．63）分／（82．86±24．50）分]差异无统计学意义（t=一0．522，P〉0．05）；②难治性抑郁症男女患者间自动思维[（81．06±20．54）分／（82．26±23．25）分]的差异无统计学意义（t=-0．284，P〉0．05）；③自动思维与HAMD总分及认识障碍因子、迟缓因子呈高度正相关性（r：0．347，P=0．000；r=0．378，P=0．000；r=0．279，P=0．003）；自动思维与HAMA总分及精神性焦虑因子呈高度正相关性（r=0．251，P=0．008；r=0．366，P=0．000）。结论难治性抑郁症与非难治性抑郁症患者自动思维的不存在差异，抑郁、焦虑症状能够增加难治性抑郁症患者自动思维出现的频率。
Objective To explore the difference of automatic thoughts between treatment-resistant depres- sion patients and non-treatment-resistant depression patients, and whether the symptoms of depression and anxiety can affect the negative automatic thoughts or not. Methods Patients were measured by automatic thoughts ques- tionnaire（ ATQ ）, Hamilton depression scale-17 （ HAMD-17 ）, Hamilton anxiety scale （HAMA）. Results ① Treatment-resistant depression patients and non-treatment-resistant depression patients had no significant difference on the total score of ATQ , which were （ 81.57 ± 21.63 ） and （ 82.86 ± 24.50 ） respectively. ②There was no signif- icant difference on the total score of ATQ between male and female in treatment-resistant depression, which were （81.06 ±20.54） and （ 82.26 ± 23.25 ） respectively. ③The correlation coefficient （ r value ） between the total score of ATQ and two subfactors （cognitive disorder and slow）scores of HAMD, total score of HAMA, mental anxi- ety factor score of HAMA in TRD group were 0. 347,0. 378,0. 279,0. 251 and 0. 366 respectively,which signifi- cantly correlated. Conclusion The difference in negative automatic thoughts characteristics of treatment-resistant depression and non-depression patients is not significant, and the symptoms of depression and anxiety can increase frequency of automatic thoughts.
