摘要
目的探讨腓肠神经营养血管逆行皮瓣修复足部缺损的临床应用效果。方法以腓肠神经及其营养血管为蒂,根据创面大小,逆行转移修复足部软组织缺损21例,观察术后皮瓣成活情况。结果 21例皮瓣全部成活,18例伤口一期愈合,1例皮瓣浅表坏死,2例伤口感染经换药治疗痊愈。供皮区创面一期愈合。随访6月至2.5年,皮瓣外形,质地,色泽良好,踝关节功能满意。结论皮瓣血供恒定,手术操作简单,不牺牲主要血管神经,皮瓣血供可靠,血运丰富,成活率高,临床上比较常用,且具有很强的抗感染能力,是修复足部外伤导致软组织缺损的理想方法。
Objective To explore the effectiveness of clinical application of the sural neurovascular reversed flap in repairing soft-tissue defection of ankle and foot. Methods According to the sizes of wound surfaces and based on the sural nerves and their nutrient vessels, 21 patients with soft-tissue defects in the ankle and foot were treated by use of the sural neurovascular reversed flap, and the postoperative survival situations of flaps were also observed. Results 21 flaps were completely survived, 18 woulds were primary healing, superficial necrosis occurred for one flap, wound infection occurred in 2 cases and healed after changing drugs. The wound surfaces of donor site of the split-skin were primary healing. All the cases were followed-up from 3 months to 2.5 years, and it was found that the circulation, color and texture of the flaps were excellent and the function of ankle joints were good. Conclusion Flaps have stable blood supply and the operation concerned is also simple, without sacrifice of the major arteries and nerves.Moreover, flaps have sufficient and reliable blood supply and a high survival rate, so this operation is often adopted clinically, which may have strong ability to resist inflection and thus is an ideal treatment to repair soft-tissue defection of ankle and foot.
