摘要
目的分析15例（15只眼）植入AcrysofReSTOR多焦点人工晶状体（MIOL）不满意原因、症状及处理方法。方法回顾分析2009年12月至2011年1月期间86例（137只眼）植入AcrysofReSTOR多焦点人工晶状体的患者，主要评估术后裸眼远、近视力，最佳矫正远、近视力，并发症及患者不满意原因。结果共有15例患者（15只眼）诉手术效果不满意，占总术眼的10．9％，其中视物稍模糊10只眼（66．7％），主要由眼底病变、10L测算误差、散光及后囊混浊等因素引起。出现视觉异常为5只眼，其中，光晕4只眼（26．7％），无法表述的不适1只眼（6．7％）。15只眼中1只眼行IOL置换，1眼行角巩膜缘松解切口散光矫正术（LRI），2只眼行后囊膜切开术，余均戴镜、观察等保守治疗，大部分患者症状缓解。结论术后未获预期视力是患者不满意的主要原因，多数患者经过对症治疗可获得满意效果。
Objective To analyze the symptoms, etiology, and treatment of 15 patients （15 eyes） dissatisfaction after AcrySof ReSTOR multifocal intraocular lens （IOL） implantation. Methods This study comprised consecutive 86 cataract patients （137 eyes） having implantation of AcrySof ReSTOR multifocal IOLs between December 2009 and January 2011. Outcomes analysis included distance and near visual acuities, complications and visual complaints. Results Fifteen patients （15 eyes, 10.9%） reported unwanted visual symptoms after multifocal IOL implantation. Ten patients （66.7%） reported blurred vision, causes of blurred vision included fimdus diseases, incorrect IOL power implanted, astigmatism and posterior capsule opacification （PCO）. Five patients reported dysphotopsia, included halo （4eyes, 26.7%） and unexplained etiology （1 eye, 6.7%）. One eye treated with IOL exchanged, One eye treated with limbal relaxing incision （LRI）, two eyes treated with Nd：YAG capsulotomy. The other patients received conservative management and most patients had improvement after these treatment. Conclusions Blurred vision is the leading cause of dissatisfaction among patients with multifocal IOLs, and many of these patients can be managed successfully with appropriate treatment.
