摘要
目的:观察益气温阳法、温肾阳法与温心阳法对冠脉结扎致充血性心力衰竭大鼠心功能及心室重构的影响。方法:通过冠脉结扎法制备充血性心力衰竭大鼠模型,50只造模成功大鼠随机分为温心阳法组、温肾阳法组、益气温阳法组、模型组以及假手术组5组,每组各10只。各药物组予相应药物灌胃8周。应用酶联免疫吸附法(ELISA)测定血清脑钠肽(BNP)水平,记录血流动力学变化,计算左室重量指数(LVMI);HE染色光镜下、透射电镜下分别观察心肌细胞病理形态结构及超微结构。结果:各治疗组大鼠BNP水平均较模型组显著下降(P<0.01),益气温阳法组及温肾阳法组治疗后血清BNP水平低于温心阳法组(P<0.05);益气温阳法组和温肾阳法组左室收缩压(LVSP)均高于温心阳法组(P<0.01);益气温阳法组降低左室舒张末压(LVEDP)疗效优于温心阳法组(P<0.05),与温肾阳法组差异无统计学意义;温肾阳法组+dp/dtmax高于温心阳法组(P<0.05),与益气温阳法组差异无统计学意义;益气温阳法组及温肾阳法组-dp/dtmax高于模型组(P<0.05),低于假手术组(P<0.05);益气温阳法组、温肾阳法组大鼠LVMI低于温心阳法组(P<0.01);三组均可改善心衰大鼠心肌细胞病理形态结构及超微结构,益气温阳法组及温肾阳法组较温心阳法组更佳。结论:益气温阳法、温肾阳法及温心阳法均可改善心梗后心力衰竭大鼠模型心脏功能,其作用机制是通过改善心肌细胞病理形态结构及超微结构,干预心室重构;温肾阳法及益气温阳法疗效更佳,提示温肾助阳是中医药治疗心力衰竭的基本大法。
Objective： To observe the effect of three warming yang methods on cardiac function and ventricular remodeling of heart failure rats after myocardial infarction.Methods： The heart failure rat model was established by ligating coronary artery,fifty rats were randomized into five groups： warming heart group,warming kidney group,invigorating qi to warm yang group,model group and sham-operation group,10 rats were in each group.After 8 weeks,the Brain natriuretic peptide（BNP） was determined by ELISA,and hemodynamic changes was determined,the left ventricular mass index（LVMI） was calculated,and myocardial ultrastructure was observed with optical microscope and electron microscopy.Results： BNP levels of treatment groups were decreased significantly as compared with those in model group（P0.01）,BNP levels in invigorating qi to warm yang group and warming kidney group were lower than those in warming heart group（P0.05）;LVSP of invigorating qi to warm yang group and warming kidney group were higher than that in warming heart group（P0.01）;LVEDP-lowering effect of invigorating qi to warm yang group was better than that in warming heart group（P0.05）,there was no significant difference in invigorating qi to warm yang group and warming kidney group.＋dp/dtmax of warming kidney group were higher than that in warming heart group（P0.05）,and there was no significant difference between invigorating qi to warm yang group and warming kidney group;＋dp/dtmax of invigorating qi to warm yang group and warming kidney group were higher than that in model group（P0.05）,lower than that in sham-operation group（P0.05）;LVMI of invigorating qi to warm yang group and warming kidney group were lower than that in warming heart group（P0.01）.The pathological morphology and ultrastructure of myocardial cells of rats with heart failure were improved in warming heart group,warming kidney and invigorating qi to warm yang group.Invigorating qi to warm yang group and warming kidney group were better than warmi
