摘要
目的探讨经腹膜前间隙行无张力股疝修补术的可行性及手术要点。方法回顾性分析河南省漯河市疝外科医院2009年5月至2011年5月采用经腹股沟切口于腹膜前间隙置治疗股疝的麻醉方式、手术时间、手术费用、术后复发、伤口疼痛、伤口愈合、术后并发症等情况。随访7d至1年,全面了解患者病情的痊愈情况与传统McVay术式患者结果进行对比。结果采用局部神经阻滞下经腹膜前间隙补片修补股疝手术时间为40～50min,术后观察2～48h,视病情稳定许可出院。手术费用平均2580元。术后未见复发、慢性疼痛。术后牵拉、异物不适感1例,术后3d恢复日常活动。McVay组手术时间为35～55min,采用的硬膜外麻醉,平均住院时间为5～7d,住院费用为3000元,术后复发2例,慢性疼痛4例,腹股沟区牵拉感15例,术后7～30d恢复日常活动,二组伤口愈合均良好。结论采用局部神经阻滞下经腹膜前间隙补片行无张力修补股疝比张力性修补(McVay法)具有麻醉方式更简捷、手术后恢复时间更短,治疗总费用有所下降等特点,是一种安全、有效的治疗方法。
Objective To investigate the feasibility of transperitoneal space tension-free femoral hernia repair and surgical points. Methods The type of anesthesia, operation time, operation cost, postoperative recurrence, wound pain, would healing, postoperative complications in treatment of femoral hernias were retrospectively assessed in the Luohe hernia surgical hospital （ Henan Province） from May 2009 to May 2011. After 7-day to one-year follow-ups, a comprehensive understanding of healing of the patients＇conditions was compared with traditional McVay surgical patient outcomes. Results The operation time for local nerve black after preperitoneal gap patch repair for femoral hernia was 40 to 50 mins. In 2 to 48 hour postoperative observation, the patients can be discharged with stable diseases. The mean of surgery costs was $ 2 580. No recurrence after surgery and chronic pain were observed. The postoperative traction and foreign body discom- fort were observed in one case. The daily activities were resumed 3 clays after operation. The operation tim was 35 to 55 rains in the McVay group. By using epidural anesthesia, the mean of length of stay for hospitalization was 5 to 7 days and the cost of hospitalization was $3 000. There were two eases of recurrence,four cases of chronic pain, and fifteen cases of groin area traction. 7 to 30 days after operation, daily activities were recov- ered and two groups of wound healing are satisfied. Conclusions Local nerve block patch for tension-free femoral hernia repair （McVay） owns a simpler anesthesia, shorter recovery time after surgery with declined the total treatment costs, which is a safe and effective method of treatment.
