摘要
【目的】探讨异丙酚复合氯胺酮用于门诊小儿口腔手术麻醉的临床效果和安全性。【方法】选择本院门诊小儿口腔手术患儿84例,随机分为观察组(n=42)与对照组(n=42),对照组给予氯胺酮肌注诱导麻醉,观察组给予氯胺酮静脉注射诱导麻醉,异丙酚维持麻醉。对比观察两组患儿麻醉不同时间点心率、麻醉起效时间、患儿清醒时间、氯胺酮总用量及呕吐等不良反应。【结果】观察组在麻醉后5min和手术结束时心率与麻醉前心率比较差异无显著性(P〉0.05)。而对照组麻醉后5min和手术结束时心率明显高于麻醉前,且差异有显著性(P〈0.05)。观察组在麻醉后5min和手术结束时心率均明显低于对照组,且差异有显著性(P〈0.05)。观察组患儿麻醉起效时间、清醒时间短于,氯胺酮总用量明显少于对照组,且差异有显著性(P〈0.05)。观察组呕吐3例,对照组呕吐12例。两组相比较差异无显著性(P〉0.05)。【结论】口腔小儿门诊手术应用异丙酚复合氯胺酮麻醉起效快,麻醉过程平稳,安全性好,效果优于单纯氯胺酮麻醉。
[Objective] To explore the clinical efficacy and safety of propofolum combined with ketamine anesthesia in pediatric outpatients undergoing 0ral operation. [Methods] A total of 84 pediatric outpatients un- dergoing oral operation in our hospital were enrolled in this study. All patients were randomly divided into the treatment group( n =42) and control group( n =42). The control group was given anesthesia induction with ketamine by intramuscular injection. The observation group was given anesthesia induction with ketamine, and then the maintenance anesthesia with propofol. Heart rate, onset time, waking time, ketamine dosage and in- cidence of vomiting were observed and compared between the two groups. [ResuhsI There was no significant difference in heart rate of the observation group between 5rain after anesthesia, the end of the surgery and be- fore anesthesia( P 〉0.05). Heart rate in control group 5rain after anesthesia and the end of surgery were ob- viously lower than those before anesthesia, and there was significant difference( P d0.05). Heart rate in the observation group 5min after anesthesia and the end of surgery were obviously lower than those in control group, and there was significant difference( P 〈0.05). The onset time of anesthesia, waking time and total dosage of ketamine in the observation group were significantly lower than those in control group, and there were significant differences( P 〈0.05). Vomiting occurred in 3 patients in the observation group and 12 pa- tients in control group, but there was no significant difference( P 〉0.05). [ConclusionI Propofolum eombined with ketamine anesthesia in pediatric outpatients undergoing oral operation has fast anesthesia onset, stable an- esthesia procedure and good safety, and its efficacy is better than that of simple ketamine anesthesia.
