摘要
目的调查分析目前衡阳市村医现状,探索解决村医存在的问题。方法采用分层整群随机抽样法抽取衡阳市13个县市区125名村医进行问卷调查。结果衡阳市农村女村医明显不足,男女村医性别比为3.7:1;村医年龄结构不合理,年龄在30岁以下的村医仅占2.40%,年龄在51岁以上的村医占46.60%,两者相差19.42倍;从医30年以上者占48%,2002年以后从事村医者数量剧减,十年内新增的村医仅占5.6%;村医学历结构偏低,无医学学历者占28.80%,中专及以上学历者只占71.20%;村医公共卫生服务经费补助标准偏低,人均4元以上的仅占9.60%,月收入在1 000元以下的占56.19%。结论衡阳市农村女村医明显不足,村医年龄结构明显偏老龄化,且学历结构与收入水平均偏低,建议尽快明确给村医的公共卫生服务劳务补助标准并适当考虑给予村医一定的保底工资,改善村医收入,加强乡村医生定期免费培训,并参照衡阳市对村干部或小学教师的相关规定,给予村医养老待遇。
Objective To survey and analyze the current status of village doctors in Hengyang,and to explore and solve the existing problem.Methods A questionnaire survey was conducted among 125 village doctors selected by stratified cluster random sampling from 13 counties（cities） in Hengyang.Results Villages in Hengyang showed a marked lack of female doctors,and the ratio of male to female doctors was 3.7：1.The doctors’ age structure was disproportionate.The percentage of doctors under 30 years old was only 2.40%,while the doctors aged 51 years or over accounted for 46.60%,there was a difference of 19.42 times between the two age groups.48% doctors had been working within their fields of specialization for over 30 years.The number of new doctors declined drastically since 2002,and only 5.6% doctors were newly employed in last decade.The educational level of village doctors was relatively low.The doctors without medical education background accounted for 28.80%,and only 71.20% had education background with technical secondary school or above.The standard of grants for public hygienic service was under the average,which meant that only 9.60% doctors received RMB 4 or above per person.And 56.19% doctors were engaged at a salary of below 1,000 RMB.Conclusions Female doctors are severely needed in rural areas of Hengyang.Aging problem is serious among the village doctors,moreover,the education level and average income are both lower.The author suggests raising the standard of grants for public hygienic service,taking minimum wage system into account,increasing the doctors’ income,strengthening the free periodic training program and introducing the social security system referring to the civic regulations on village cadres or elementary school teachers.
