长学制医学生内科临床实践的改革和模式研究 被引量:2

摘要 目的探讨结合"早期接触临床"和"以问题为基础的学习"(problem-based learning,PBL)的教学模式在长学制医学生内科临床实践中的应用,旨在探索一种有效提高长学制医学生临床实际应用能力的教学模式。方法将西安交通大学医学院第二附属医院2008级七年制(第7学期)临床医学系1、2班学生,随机分为实验组和对照组。实验组学生采用"早期接触临床"和"PBL"相结合的教学模式,对照组学生采用传统教学模式,比较分析两组教学效果的差异。结果实验组与对照组相比,基础理论知识成绩无显著性差异[(38.55±4.86)vs(37.59±5.73),P>0.05],但病例分析考试成绩及总成绩实验组均明显优于对照组[分别为(39.75±3.19)vs(34.18±4.53),P<0.01和(78.30±7.15)vs(71.76±9.09),P<0.05]。教学效果反馈意见显示实验组的积极性评价明显高于对照组(P<0.05)。结论 "早期接触临床"和"PBL"相结合的教学模式优于传统教学模式,更能提高医学生临床实际应用能力。 Objective To explore an effective teaching mode for improving the clinical application ability of internal medicine, this study was designed to invest the teaching mode of combining early contact of clinic and problem-based learning (PBL) in the clinical practice of internal medicine for long schooling medical students. Methods Class 1 and 2 were randomly divided into experimental group and control group from the seven-year clinical medicine of grade 2008(the 7 semester), the Second Affiliated Hospital of Medical School, Xi' an Jiaotong University. Students in the experimental group used the teaching mode of combining early contact of clinic and PBL and in the control group used the traditional teaching mode in order to compare the difference in teaching effect between the two groups. Results There was no significant difference in basic theoretical knowledge scores between the experimental group and control group [(38. 55±4. 86) vs (37. 59±5.73), P〉0. 05]. But the case analysis scores and the total scores of the experimental group were significantly higher than the experimental group [(39.75±3.19) vs (34. 18±4. 53), P〈0. 01 and (78. 30±7. 15) vs (71.76±9.09), P〈0. 05, respectively]. Feedback of teaching effect from students showed that the positive evaluations of the experimental group were significantly higher than the control group (P〈0. 05). Conclusion The teaching mode of combining early contact of clinic and PBL is superior to the traditional teaching mode, and better to improve the practical application of clinical ability for medical students.
