摘要
目的探讨帕瑞昔布预防全身麻醉术后躁动的有效性。方法选择2010年9月至2011年12月该院收治择期行鼻窦镜手术的患者90例,随机分为3组,每组30例,3组均在常规治疗的基础上,帕瑞昔布组静脉注射帕瑞昔布40mg,芬太尼组静脉注射芬太尼1μg/kg,对照组静脉注射生理盐水5mL。比较3组患者的术后情况。结果帕瑞昔布组和芬太尼组给药后不同时间点疼痛视觉模拟(VAS)评分低于对照组(P<0.05),帕瑞昔布组的镇静评分与芬太尼组比较,差异有统计学意义(P<0.05),8h以后3组患者躁动、镇静评分差异均无统计学意义(P>0.05),帕瑞昔布组给药后2、4、8h恶心、呕吐评分与芬太尼组、对照组比较,差异有统计学意义(P<0.05),12h后3组比较差异无统计学意义。结论静脉注射帕瑞昔布40mg,能够使患者术后躁动、恶心、呕吐的发生率大大降低,并且不会造成患者术后的过度镇静。
Objective To observe the effectiveness of parecoxib for preventing postoperative dysphoria in general anesthesia.Methods 90 patients with endoscopic sinus surgery treated in this hospital from September 2010 to November 2011 were selected and randomly divided into three groups,30 cases in each group.On the basis of the routine therapy,the parecoxib group（T） was intravenously injected by parecoxib 40 mg,the fentanyl group（F） by fentanyl 1 μg/kg and the normal saline group（C） by normal saline 5 mL.The postoperative conditions were compared among 3 groups.Results The VAS scores at different postoperative time points in the group T and F were lower than those in the control group（P0.05）.The sedative score in the group T had statistical difference as compared with the group F（P0.05）.The dysphoria and sedative scores after 8 h showed no statistical difference among 3 groups（P0.05）.The nausea and vomiting scores at 2,4,8 h after drug administration in the group T had statistical difference compared with the group F and C（P0.05）,but no statistical difference was found after 12 h in the comparison of 3 groups.Conclusion Intravenous injection of parecoxib 40 mg can reduce the incidence rate of postoperative dysphoria,vomiting and nausea in the patients with endoscopic sinus surgery,in addition,this dosage could not lead to postoperative over sedation.
