双重滤过血浆置换降低高群体反应性抗体的研究 被引量:5

摘要 目的与单纯血浆置换对比,双重滤过血浆置换仅需少量白蛋白即可以对致病性抗体进行清除,包括高群体反应性抗体(PRA)。方法选择在309医院拟行肾移植手术高PRA患者13例,肾移植术后急性排异合并高PRA患者3例,进行双重滤过血浆置换治疗152例次,每例患者治疗前后检验PRA等指标。结果与治疗前PRA(56.72±4.53)%相比,治疗后PRA(<10%)显著下降(P<0.05),7例高PRA患者接受肾移植手术未出现急性排异,3例肾移植术后急性排异合并高PRA患者脱离透析。结论双重滤过血浆置换能降低高群体反应性抗体水平,减少肾移植术前及术后急性排异反应。 Objective Comparision with simple plasma exchange,double filtration plasmapheresis needs only small amount of albumin to reduce pathogenic antibodies,include high panel reative antibodies(PRA).Methods 13 patients with high PRA waiting for kidney transplanting and 3 kidney transplanting patients with acute rejection combined with high PRA in 309 Hospital were selected to receive double filtration plasmapheresis treatments.PRA、 immune globulin,hemoglobin,and creatinine level of each patient were detected before and after treatment.Results Comparing with the PRA(56.72±4.53)% before treatment,the PRA(10%)after treatment was significantly decreased(P〈0.05),7 patients with high PRA levels did not have acute rejection after renal transplanting,3 renal transplant patients occurring acute rejection and with high PRA got rid of the dialysis.Conclusion The double filtration plasmapheresis can decrease the high PRA level,and reduce the risks of acute rejection before ang after renal transplanting.
