摘要 目的:研究固定义齿长桥受载后的生物力学分析。方法:采用CT扫描获取健康成人上颌牙及其支持组织的二维图像,通过计算机重建技术建立三维立体图像并获得了上颌前牙区牙列缺损后的双侧两基牙双端固定桥的三维有限元数学模型,在相同垂直载荷和水平载荷下分析了双侧两基牙牙槽骨高度对固定桥应力分布的影响。结果:当双侧两基牙牙槽骨无吸收时固定桥双侧两基牙应力分布基本均匀;当双侧两基牙牙槽骨同时吸收大于35%时,固定桥双侧两基牙开始出现应力集中;当双侧近中基牙牙槽骨同时吸收大于10%、远中基牙牙槽骨无吸收时,固定桥双侧近中基牙开始出现应力集中;当双侧远中基牙牙槽骨同时吸收大于25%、近中基牙牙槽骨无吸收时,固定桥双侧远中基牙开始出现应力集中。结论:固定义齿长桥受载后的应力分布情况较为复杂,其中双侧近中基牙牙槽骨同时吸收对基牙应力分布的影响最大。 Objective: To study the stress distribution around the abutment teeth of long rigid fixed bridges. Methods: Two - dimensional images of mandibular bone, teeth and periodontal supporting tissue of healthy people were obtained by spiral CT. Three - dimensional digital model were developed by imaging software. Then the three - dimensional finite element model of bilateral abutments of long fixed bridge of maxillary anterior teeth was developed. Under the same vertical and horizontal load, effects of different height of alveolar bone on the stress distribution around the abutment teeth of fixed bridge were analyzed. Results: When there was no alveolar bone absorption around the bilateral abutments, the stress showed distributing evenly without stress concentration. When the alveolar bone absorption around the bilateral abutments was above 35%, stress concentration appeared in the abutments. When the alveolar bone absorption around the bilateral mesial abutments reached 10% and no bone absorption around the bilateral distal abutments, stress concentration appeared in the bilateral mesial abutments. When the alveolar bone absorption around the distal abutments reached 25% and no bone absorption around the mesial abutments, stress concentration appeared in the bilateral distal abutments. Conclusions: Stress distribution of long rigid fixed bridge is rather complicated. Influence of bone absorption around the bilateral mesial abutments on the stress distribution around the bridge abutments showed the most severe.
