自体腘绳肌腱与异体胫前肌腱重建前交叉韧带短期疗效比较 被引量:7

Anterior Cruciate Ligament Reconstruction with Hamstring Tendon Autografts Versus Anterior Tibial Muscle Tendon Allografts: A Comparative Study

摘要 目的:探讨关节镜下应用自体腘绳肌腱与异体胫前肌腱重建前交叉韧带术后的短期疗效。方法:选择2009年8月-2011年2月在本院行前交叉韧带重建术并具有完整随访资料的患者50例,其中应用自体腘绳肌腱患者24例,应用异体胫前肌腱患者26例,术后随访指标包括前抽屉试验、轴移试验、髌上10cm大腿周径、主动屈膝角度、有无膝前痛、局部压痛、感觉减退;同时,采用Lysholm评分表、Tegner评分表、IKDC膝关节主观评价表评价患者主观运动功能。结果:两组患者在前抽屉试验、轴移试验、髌上10cm大腿周径、有无膝前痛、局部压痛、感觉减退、Lysholm评分、Tegner评分、IKDC膝关节主观功能评分各方面均无明显差别(P<0.05),而异体胫前肌腱组在主动屈膝角度上大于自体腘绳肌腱组(P<0.05),受伤前后患者Tegner评分有明显差别(P<0.05)。结论:应用自体腘绳肌腱和异体肌腱重建前交叉韧带均可获得满意的短期疗效。 Objective: To compare the outcomes of hamstring tendon autografts versus anterior tibial muscle tendon allografts under arthroscopy in the treatment of anterior cruciate ligament recon- struction. Methods: Fifty cases of anterior cruciate ligament (ACL) rupture treated with ham- string tendon autografts (24 patients) or with anterior tibial muscle tendon allografts (26 pa- tients) from August 2009 to February 2011 in Zhongnan Hospital of Wuhan University were ret- rospectively studied. Anterior drawer test, Pivot Shift Test, leg circumference 10 cm up the pa- tella, active knee flexion angle, anterior knee pain, local tenderness, numbness, Lysholm scores, Tegner scores and International Knee Documentation Committee (IKDC) scores were used to evaluate the clinical results. Results: There was no statistically significant difference between the two groups in anterior drawer test, Pivot Shift Test, leg circumference 10 cm up the patella, an- terior knee pain,local tenderness, numbness, IKDC scores, Tegner scores, or Lysholm scores (P 〈0.05). The allografts group had a better active knee flexion angle than autografts group (P〈 0.05), and there was a statistically significant difference in Tegner scores between pre-surgery and post-surgery (P〈0.05). Conclusion: ACL reconstruction with hamstring tendon autograftshad a similar excellent results versus anterior tibial muscle tendon allografts.
