摘要 目的讨论内镜黏膜切除术(EMR)治疗食管黏膜下肿瘤的临床价值。方法收集经超声内镜诊断为直径小于2cm食管黏膜肌层和黏膜下层肿瘤73例患者应用EMR治疗,术后内镜随访。结果 73例患者共76个病变,完全切除75个病灶,完全切除率为98.7%(75/76)。术中有5例(6.8%)患者渗血,无穿孔及术后出血等严重并发症。术后72例患者完成了1~3个月内镜随访,术后创面均完全愈合(100%),随访期无食管狭窄及局部复发。结论 EMR对直径小于2cm起源于黏膜肌层及黏膜下层的食管黏膜下肿瘤,能一次性完全切除,并能获取完整病理诊断。 【Objective】To evaluate the value of endoscopic mucosal resection (EMR) for treatment of esophageal submucosal tumors(SMTs).【Methods】76 esophageal submucosal tumors less than 2cm of diameter from muscularis and submucosa of 73 patients were confirmed by endoscopic ultrasonography (EUS), all lesions were treated with EMR and followed up with endoscopy examination. 【Results】75 lesions were successfully treated with EMR, and the success rate was 98.7%(65/66). Five cases (6.8%) were complicated with bleeding, during the procedure, none cases suffered from perforation and bleeding in post-operation. 72 patients were followed up with endoscopy examination from1 to 3 months. The wound surfaces were completely cured, no case suffered from recurrence and stenosis during the follow -up. 【Conclusion】EMR could treat esophageal submucosal tumors less than 2cm of diameter which come from muscularis and submucosa completely and provide complete pathologic data.
