摘要
目的探讨临床护理路径及康复训练表在治疗人工全膝关节置换术（TKA）患者中的应用效果。方法将2011年1月至2012年3月行TKA的100例患者随机分为对照组和观察组，每组各50例。对照组采用常规传统的护理及常规锻炼方法，观察组采用临床护理路径的护理及应用康复训练表进行康复锻炼。出院时接受膝关节康复测评（HSS）、护理工作满意度、健康教育知识知晓率、功能锻炼情况、并发症的发生率、日常生活活动能力和住院天数的调查。结果观察组和对照组的膝关节HSS评分满意（42比27例），对护理工作的满意度（98％比84％），康复知识知晓的达标率（100％比82％），功能锻炼方法满意（43比31例），日常生活活动能力满意（38比16例），观察组均高于对照组，差异有统计学意义（P〈0．01）。并发症的发生率（2％比24％）和平均住院天数[（10．66±2．07）比（12．57±2．96）d]，观察组均低于对照组，差异有统计学意义（P〈0．01）。结论应用临床护理路径及康复训练表能明显提高患者生活质量i保证了护理质量。
Objective To explore the clinical effect of clinical nursing path （CNP） and rehabilitation training table in the treatment of limbs fracture. Methods Totals of 90 patients with artificial total knee replacement were randomly divided into control group and experimental group. The control group received the conventional traditional nursing and regular exercise method, while the experimental group adopted clinical nursing path nursing and rehabilitation exercise according to the rehabilitation training application table. Then, the satisfaction for nursing work, knowledge about health education, accuracy of functional exercise, the incidence of complications, activities of daily living and length of stay of the patients were observed. Results After intervention, the hospital satisfaction cases for special surgery （42 vs 27 ）, satisfaction scale for nursing work （98% vs 84% ）, knowledge about health education （ 100% vs 82% ）, accuracy of functional exercise （86% vs 62% ） of experimental group were significantly better than that of control group （ P 〈 0.01 ）. And the incidence of complications（2% vs 24% ） and activities of daily living [ （ 10.66 ± 2.07 ） d vs （ 12.57 ± 2.96） d] of experimental group were obviously lower than that of control group （P 〈 0.01 ）. Conclusions The application of CNP and rehabilitation training table in the patients with artificial total knee replacement can improve their quality of life, and make sure the nursing quality.
