摘要 目的分析无创动脉检测技术在2型糖尿病合并下肢血管病变的应用价值。方法选择2007年1月~2008年9月治疗的2型糖尿病患者56例为糖尿病组,选择同期健康体检人员60例为对照组,检测脉搏波传导速度(PWV)、踝臂指数(ABI)及脉压指数,对糖尿病组下肢动脉斑块情况进行统计。结果糖尿病组PWV[(19.08±5.16)m/s]高于对照组[(15.40±3.36)m/s],糖尿病组ABI(1.06±0.11)低于对照组(1.14±0.08),糖尿病组脉压指数(0.42±0.04)高于对照组(0.34±0.05),差异均有高度统计学意义(均P<0.01)。其中7例患者ABI<0.9,进一步行下肢动脉彩超发现5例患者有动脉斑块形成。结论无创动脉检测技术对2型糖尿病合并下肢血管病变患者有较好的敏感性,能早期发现动脉硬化和可能存在动脉阻塞患者,为临床早期干预治疗提供较好的应用价值。 Objective To analyze the application value of non-invasive atherosclerosis detection technology in type 2 diabetic patients with lower extremity vascular disease. Methods 56 patients with type 2 diabetes from January 2007 to September 2008 were selected as diabetes mellitus group, 60 cases of healthy personnel were chosen as control group; pulse wave speed (PWV), ankle-brachial index (ABI) and pulse pressure index were detected, lower limb artery plaque situation of diabetes mellitus group was analyzed. Results PWV in diabetes mellitus group [(19.08±5.16) m/s] was higher than that in control group [(15.40±3.36) m/s], ABI in diabetes mellitus group (1.06±0.11) was lower than that in control group (1.14±0.08), pulse pressure index in diabetes mellitus group (0.42±0.04) was higher than that in control group (0.34±0.05), the differences were statistically significant (P 〈 0.01). 5 cases with artery plaque formation were found by color Doppler ultrasound in 7 cases whose ABI was less than 0.9. Conclusion Non-invasive atheroscle- rosis detection technology has better sensitivity to the early detection of type 2 diabetes patients with atherosclerosis, and can find patients with blocked arteries earlier, provides better value for early clinical intervention.
