摘要
目的探讨十二指肠憩室的多层螺旋CT(MSCT)诊断及其临床意义,以提高放射科医生对其认识。方法对28例经临床证实的十二指肠憩室患者的MSCT影像资料进行归纳,分析其影像学表现及临床意义。结果 28例患者共检出憩室33个,位于十二指肠降段28个,位于十二指肠水平段5个。典型的十二指肠憩室在CT上表现为胰头右后方紧贴降段内侧壁的大小不一、圆形或类圆形囊袋影,21个囊内表现为气-液平面,8个囊内表现为混杂密度影,4个囊内仅为气体密度影;检查前服用阳性对比剂者均能见阳性对比剂进入憩室,其中囊壁增厚伴边缘模糊2例,并发胆总管扩张7例,胆总管结石3例,胆囊结石3例,急性胰腺炎1例,胆囊炎2例。结论 MSCT扫描对十二指肠憩室具有较高的诊断准确性,并能及时发现胆、胰疾病及其并发症,对判断二者的临床关系具有重要指导作用,放射科医生应重视并提高对十二指肠憩室的诊断。
Objective To explore the diagnosis of multislice spiral CT（MSCT） in duodenal diverticula and its clinical sig- nificance to increase the the radiologists＇ cognition on it. Methods The MSCT imaging and clinical data in 28 cases of duodenal diverticula were summarized for analyzing its imagiological manifestations and the clinical significance. Results In 28 patients, 33 duodena] diverticula were detected;28 diverticula were located in descending duodenum and only 5 diverticula were located in horizontal duodenum. The typical duodenal diverticulum on CT showed that the pancreatic head was right down to the rear section of the inside wall with different size and round or quasi-circular cystbag shadow. 21 diverticula showed fluid-air level within sac, 8 diverticula showed mixed density shadow within sac and 4 diverticula were gas density shadow within sac. Taking oral contrast a- gent before examination,the positive contrast agent was seen entering the diverticula,and 2 cases was sac wall thickening with edge blurt. Complicating choledochectasia was in 7 cases, choleducholithiasis in 3 cases, cholecystolithiasis in 3 cases, pancreati- tis in 1 case and and cholecystitis in 2 cases. Conclusion MSCT has the high accuracy for diagnosing duodenal diverticula and can timely discover pancreaticobiliary disease and its complications,which can play an important guidance role in judging the clinical relation between them. Radiologists should pay more attention to and increase the diagnosis of duodenal diverticula.
