摘要
目的探讨导致剖宫产术后切口感染的因素及干预措施,降低感染率。方法对1999年2月-2012年4月行剖宫产手术的产妇临床资料进行回顾性分析,将发生切口感染的23例产妇作为感染组,选取同期未发生术后切口感染的100例产妇作为非感染组,分析术后切口感染的相关因素。结果单因素检验显示,年龄、体重指数、手术季节为夏季、合并有基础疾病、备皮至手术时间、急诊手术、主刀医师年资、切口长度、手术时间、术中出血量、阴道指诊次数均是导致剖宫产术后切口感染的危险因素(P<0.05);多因素logistic回归分析结果显示,合并有基础疾病、备皮至手术时间长、切口长度长、手术时间长、术中出血量多、阴道指诊次数多等因素是导致术后切口感染的独立危险因素。结论导致剖宫产术后切口感染的因素较多,是多因素综合作用所致,应针对以上危险因素进行护理干预,以控制和减少剖宫产术后切口感染的发生。
OBJECTIVE To explore the risk factors for the incision infections after the cesarean section and to develop the nursing interventions so as to reduce the infection rate. METHODS The clinical records of the maternal who underwent the cesarean section from Feb 1999 to Apt 2012 were retrospectively analyzed, and totally 23 cases of maternals with incision infections were selected as the infection group, and at the same period 100 cases of maternals without incision infections were set as the non-infection group. The risk factors associated with the incision infections were analyzed. RESULTS The unlvariate analysis showed that the age, body mass index, operation in summer, complication of underlying diseases, interval between skin preparation and operation, emergency surgery, seniority of the surgeon, incision length, operation duration, intraoperative hlood loss, and frequency of vaginal dow diagnosis were the risk factors for the incision infections after the cesareans section. The multivariate logistics regression analysis revealed that the complication of underlying diseases, long interval between skin preparation and operation, long incision length, long operation duration, large amount of intraoperative blood loss, and frequent vaginal doe diagnosis were the independent risk factors for the incision infections after the cesarean section. CONCLUSION There are many factors leading to the incision infections after the cesarean section, which is due to the combined effect of multi-factor. It is necessary to perform the nursing interventions aiming at the risk factors so as to reduce the incidence of incision infections after the cesarean section.
