摘要
选取妊娠期糖尿病(GDM)孕妇(81例)与正常对照孕妇(86例),分别检测孕妇及其胎儿脐血的血糖、胰岛素,并计算胰岛素抵抗指数(HOMA—IR)。GDM孕妇空腹血清胰岛素(24.79±1.70)mU/L,胰岛素抵抗指数(5.46±0.48)均显著高于正常对照组。GDM脐血血清胰岛素(15.08±1.52)mU/L,胰岛素抵抗指数(2.80±0.35)显著高于正常对照组。GDM母亲胰岛素与脐血胰岛素呈正相关(r=0.326,P〈0.05);与脐血胰岛素抵抗指数呈正相关(r=0.378,P〈0.05)。GDM母亲子代在胎儿期已发生胰岛素抵抗,且胎儿胰岛素抵抗的发生受母亲胰岛素水平的影响。
To explore the relationship between maternal insulin levels and fetal insulin resistance. Maternal blood and venous cord blood samples were collected in gestational diabetes mellitus (GDM) mothers and control subjects. The glucose and insulin levels were measured and insulin resistance levels estimated. Maternal levels of insulin and homeostasis model of assessment for insulin resistance index ( HOMA- IR) were significantly higher in the GDM group than those in the control group (P 〈 0. 05); fetal levels of insulin and HOMA-IR were significantly higher in the GDM group than in the control group ( P 〈 0. 05). Maternal insulin level positively correlated with fetal insulin ( r = 0. 326, P 〈 0. 05) and HOMA-IR levels ( r = 0. 378, P 〈 0. 05). In this study, a higher level of fetal insulin resistance was reported in the GDM group. And maternal hyperinsulinemia might affect fetal insulin resistance in pregnant women with GDM.
