摘要 目的:对比分析多排螺旋CT冠脉成像、心电图及冠状动脉造影对冠心病临床诊断价值。方法:选取2009年1月-2011年1月笔者所在医院收治的60例疑似或确诊冠心病患者为研究对象,其中年龄大于60岁34例,为老年组;年龄小于60岁26例,为非老年组。对比两组患者的临床症状,同时给予甘油三酯、血清总胆固醇、心电图改变以及高密度脂蛋白检查,对检查结果进行分析,最终确定19例患者(老年组患者11例,非老年组患者8例)57支血管进行常规冠脉造影和MSCT检查。结果:与非老年组患者相比,老年组患者在血糖、血压以及血脂方面出现异常,并且心悸、胸闷和胸痛的临床症状较为常见。老年组患者经MSCT检查发现26支血管狭窄和冠脉造影结果一致,其敏感性83%,特异性100%,阴性预测值95%,阳性预测值93%。冠脉造影和MSCT检查结果总体一致。结论:在控制患者心率前提下,采用MSCT检查方法作为诊断患者是否患冠心病,很大程度上提高了冠脉造影诊断冠心病的符合率。 Objective: The comparative analysis of multi-slice spiral CT coronary imaging, ECG and coronary angiography of coronary heart disease clinical diagnostic value.Method: Selected from January 2009 to January 2011 in our hospital 60 cases of suspected or confirmed coronary heart disease patients for the study, of which 34 cases were older than 60 years old, as old age group; younger than the age of 60 and 26 cases, non-elderly group.Comparing the two groups of patients with clinical symptoms, while giving triglycerides, total serum cholesterol, ECG changes, and high-density lipoprotein checking, analysis of inspection results, and ultimately determine the patients ( 19 cases, 11 cases of elderly patients, non-alderly group 8 patients ), 57 vascular conventional coronary angiography and MSCT.Result: Compared with non-elderly patients, eiderly patients with abnormal blood sugar, blood pressure, and blood lipids, and heart palpitations, the more common clinical symptoms of chest tightness and chest pain.Elderly patients by MSCT was found 26 vascular stenosis and coronary angiography results, sensitivity 83%, specificity 100%, negative speculated that the value of 95%, positive an estimated value of 93%.Coronary angiography and MSCT results generally consistent.Conclusion: MSCT is under the control of heart rate in patients premise, whether as a diagnosis in patients suffering from coronary heart disease, and greatly improved the compliance rate for coronary angiography in the diagnosis of coronary artery disease.
