双下肢使用弹力绷带预防剖宫产椎管内麻醉后低血压的效果 被引量:4

Clinical Observation of Both-legs Wrapping with Elasticated Bandages in Prevention of Hypotension Following Spinal Anaesthesia for Caesarean Section

摘要 目的观察双下肢使用弹力绷带对剖宫产椎管内麻醉后低血压的预防效果。方法便利选取2011年9月至2012年2月的100例择期剖宫产产妇,按手术日期单双日分为对照组和观察组各50例,麻醉前静脉输注羟乙基淀粉500ml,观察组椎管内麻醉后给予双下肢弹力绷带包扎,对照组未予处理;若发生低血压则给予静脉注射麻黄碱5mg,可重复使用。分别记录两组产妇麻醉后低血压发生率、最低收缩压、麻黄碱用量和产妇恶心呕吐反应等,胎儿分娩后行新生儿Apgar评分。结果对照组有32例(64%)、观察组有21例(42%)发生低血压,组间差异有统计学意义(P<0.05);观察组术中最低收缩压高于对照组,而麻黄碱用量和产妇恶心呕吐发生率则低于对照组(P<0.05);两组新生儿体质量及Apgar评分差异无统计学意义(P>0.05)。结论剖宫产椎管内麻醉后双下肢使用弹力绷带,能减少椎管内麻醉后低血压发生率和减轻低血压程度。 Objective To observe the effect of wrapping of bilateral lower limbs with elasticated bandages on prevention of maternal hypotension during spinal anesthesia for cesarean section. Methods From September 2011 to February 2012,100 parturients scheduled for elective cesarean section were assigned to control group(n=50) and observational group(n:50) according to the even-numbered day and odd-num- bered day of operation date. All the parturients received intravenous hydroxyethyl starch 500 ml prior to spinal injection. After spinal injection, parturients in the observational group had their both legs wrapped with elastieated bandages,whereas parturients in the control group had not. Hypotension was treated with intravenous 5 mg ephedrine bolus doses,which was repeated as needed. The incidences of hypotension,the minimum systolic arterial pressure, the dose of ephedrine used, nausea and/or vomiting were recorded. After delivery,the Apgar scores were assessed. Results Totally,32(64%) parturients in the control group and 21 (42%) in the observational group had one or more episode of hypotension and the difference between the groups was statistically significant(P〈0.05). The minimum recorded systolic arterial pres- sure was significantly higher, while the dose of ephedrine used and the incidence of nausea and/or vomiting were significantly lower in the observational group as compared with the control group(P〈0.05). No significant difference was detected on Apgar scores and newborn weight between the two groups (P 〉0. 05). Conclusion Both-leg wrapping with elastieated bandages after spinal anesthesia can reduce the incidence and alleviate the degree of maternal hypotension associated with spinal anesthesia for cesarean section.
