两种不同激光辅助孵化方法在卵裂期冻融胚胎移植中的比较研究 被引量:13

A Comparative Study between Two Different Laser Assisted Hatching Methods in the Vitrified-Warmed Embryo Transfer at the Cleavage Stage

摘要 目的:通过比较激光透明带打孔和激光透明带薄化辅助孵化方法对卵裂期冻融胚胎移植的临床妊娠率和种植率的影响,探讨激光薄化法取代传统的激光打孔法的可能性。方法:选择在云南省第一人民医院生殖医学中心行胚胎冷冻-解冻的554个周期,对269个冻融周期行激光打孔辅助孵化(激光打孔组),对285个周期行激光薄化辅助孵化(激光薄化组)。对两组冻融胚胎不同辅助孵化方法的移植及妊娠情况、及不同年龄(≥35岁和<35岁)和不同胚胎评分(<5分、5~7分、>7分)的临床妊娠率进行比较。结果:两组解冻后胚胎的复苏率、完整胚胎率、移植胚胎形态学评分、移植胚胎数、临床妊娠率、胚胎种植率和流产率比较,差异均无统计学意义(P>0.05);不同年龄和不同胚胎评分的临床妊娠率比较,差异也无统计学意义(P>0.05)。结论:激光薄化法与激光打孔法两种激光辅助孵化方法在卵裂期冻融胚胎移植中的临床效果相近,激光薄化法在减少激光热效应对胚胎的损伤上更具有优势。 Objective: To compare the impact of zona drilling laser assisted hatching (LAH) and zona thinning LAH on the clinical pregnancy rate and implantation rate of the vitrified-warmed embryo transfer at the cleavage stage and to discuss the possibility of using zona thinning LAH instead of zona drilling LAH. Methods:A total of 554 vitrified-warmed cycles using LAH were undertaken in our centre. These cycles were divided into two groups: the zona drilling group ( n = 269) and zona thinning group ( n = 285). Clinicalpregnancy rate, implantation rate, age ( ≥ 35 and 〈 35 ) and embryo grades ( 〈 5,5 -7, 〉 7 ) between zona drilling group and zona thinning group were compared between these two groups. Results= No significant differences were noted in survival rate, complete embryo rate,clinical pregnancy rate,implantation rate and abortion rates between these two groups( P 〉0.05). Also,there are no significant differences in clinical pregnancy rate on the basis of different ages and embryo grades between these two groups( P 〉 0.05). Conclusions=There is a non-significant difference of clinical results between these two LAH groups in the vitrified-warmed embryo transfer at the cleavage stage. The zona thinning is superior to the zona breaching on reducing laser heat effect on the damage of the embryos.
