摘要
目的探讨经前会阴超低位直肠前切除(APPEAR)术在低位直肠癌保肛手术中的应用效果。方法回顾性分析APPEAR术的8例低位直肠癌患者的临床资料。结果 8例患者均顺利完成保肛手术,所有患者术后2个月后排便控便功能均恢复良好。无手术相关死亡。平均手术时间(146.3±40.5)min,术中出血量(106.8±64.1)ml,下切缘及环周切缘均阴性。术后发生吻合口瘘伴会阴部切口延迟愈合1例,会阴部切口感染1例,肠梗阻1例,均经过保守治疗后痊愈。结论APPEAR手术在低位直肠癌保肛手术中安全可靠,可提高低位直肠癌保肛率。
Objective To summarize the outcomes of anterior perineal plan E for ultra-low anterior resection of the rectum（APPEAR） for low rectal cancer.Methods Data of 8 cases with low rectal cancer underwent APPEAR were analyzed retrospectively.Results The sphincter-saving surgery was performed successfully in 8 cases with well recovery of defecation control function within two months after operation.The mean operation time was（146.3±40.5） minutes and mean blood loss was（106.8±64.1） ml.The distal rectal and mesorectal margins of the resection were both negative for cancer cells.The complication included leakage and delayed union of pereneal incision in 1 case,infection of pereneal incision in 1 case and intestinal obstruction in 1 case,which were all healed uneventfully by conservative treatment.There was no surgery-related death.Conclusion APPEAR is a safe and feasible surgical approach with higher sphincter-saving rate in the patients with low rectal cancer.
