餐后大步走有氧运动法对初治肥胖2型糖尿病患者临床疗效的影响 被引量:4

Effect of postprandial aerobic exercise walking on newly therapeutic obese patients with type 2 diabetes mellitus

摘要 目的 探讨餐后大步走有氧运动法对初治肥胖的2型糖尿病患者的糖脂代谢及胰岛素抵抗和瘦素的影响.方法 将94例初治肥胖2型糖尿病患者随机分为运动组(52例)和对照组(42例),对照组给予单纯糖尿病饮食治疗;运动组在糖尿病饮食治疗的基础上,同时给予餐后有氧大步走,每日2~3次.6个月后,78例患者完成了观察,运动组42例,对照组36例,比较两组患者干预前后糖脂代谢及其各项指标的变化的因素.结果 干预治疗3个月,以血糖、血脂下降为主.经过6个月的干预后,运动组和对照组的空腹血糖[ (6.03±1.11) mmol/L,(7.04±1.32) mmol/L]、餐后2h血糖[(8.35±1.53) mmol/L,(12.54±2.01) mmol/L]、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度胆固醇、空腹胰岛素[(13.07±1.42) uIU/ml,(14.02±2.49) uIU/ml]、餐后2h胰岛素[(36.98±2.52) uIU/ml,( 38.46±3.54)uIU/ml]、体质量指数(BMI)、腰臀比(WHR)、胰岛素敏感性(IS)(-4.54 ±0.41,-4.75±0.50)、瘦素均存在代谢与时间效应(均P<0.05或P<0.01).结论 饮食控制联合餐后有氧大步走对糖尿病的治疗,具有即时或短期改善糖脂代谢效应和长期改善BMI、WHR、IS、瘦素敏感性的作用. Objective To investigate the effect of postprandial aerobic exercise walking on glucolipid metabolism,insulin resistence and leptin of newly therapeutic obese patients with type 2 diabetes mellitus (T2DM).Methods 94 cases with newly therapeutic obese patients with T2DM were divided randomly into aerobic exercise group (52 cases) and control group(42 cases),the control group was given diet therapy only,and the aerobic exercise group was given diet therapy and aerobic exercise intervention,2-3 times per day for six months.78 cases completed the study (42 cases in aerobic exercise group and 36 cases in control group).Each index of change in the two groups was compared.Results After six months intervention,the main therapeutic effect was the decline of glucose and lipid.The change of FPG( (6.03 ± 1.11 ) mmol/L,(7.04 ± 1.32) mmol/L),2hPG( (8.35 ± 1.53 )mmol/L,( 12.54 ± 2.01 ) mmol/L),TG,TC,LDL-C,HDL-C,FINS ( ( 13.07 ± 1.42 ) uIU/ml,( 14.02 ± 2.49 )uIU/ml ),2 hPINS ( ( 36.98 ± 2.52 ) uIU/ml,( 38.46 ± 3.54 ) uIU/ml),BMI,WHR,ISI ( - 4.54 ± 0.41,- 4.75 ±0.50) and leptin was found metabolism and time effect between two groups before and after intervention.The change was respectively(P 〈 0.05 or P 〈 0.01 ).Conclusion The diet control combined with postprandial aerobic exercise walking therapy has real-time or short-term improving glucolipid metabolism effect and long-term improving the BMI,WHR,ISI and leptin sensitivity effect.
