摘要
目的探讨PDCA循环管理模式在肝移植患者中应用的效果。方法选择2009年4月至2011年5月接受肝移植手术的患者120例，按就诊时间分为实验组和对照组各60例，对照组应用常规管理模式，实验组应用PDCA循环管理模式，于实施术前及术后第1次复查时发放自行设计的肝移植患者依从性调查问卷、生活质量指数评定量表和对护士工作的满意度量表，除去4例失访人员，比较两组结果。结果实验组患者在服药、生活习惯、合理锻炼、自我监测护理方面依从较好者分别为57，52，51，52例，总体生活质量评分为（6．52±0．53）分，满意率为94．92％，均高于对照组[44，39，40，40例，（4．36±0．78）分，82．46％]，差异均有统计学意义（依从性χ2分别为6．3096，6．6646，4．5365，5．6991，满意度χ2=4．5225，生活质量t=17．498；P〈0．05）；两组患者在生活质量4个方面评分比较差异均有统计学意义（P〈0．05）。结论应用PDCA循环管理模式能提高肝移植患者的依从性和生活质量，增进患者对护理工作的满意度。
Objective To investigate the application of PDCA circulation in the patients undergoing liver transplantation. Methods Apart from 1 of experiment and 3 of control group failed to be followed up, 116 cases undergoing liver transplantation in our liver transplant center from April 2009 to May 2011 were divided into the experiment group （ n = 59 ） and control group （ n = 57 ） by registration order. PDCA circulation was used in the experiment group, but in the control group, routine management mode was used. A self-designed questionnaire about the compliance of liver transplantation and the Satisfaction Degree of Patients to the Nursing Work and Quality of Life Index （QL-Index） were used to investigate at the first follow-up time. Results There was a significant difference between two groups in the aspects of habits and customs, drug-taking compliance, reasonable physical training, self-monitoring and the satisfaction degree of patients to the nursing work [ 57 vs 44,52 vs 39,51 vs 40,52 vs 40; 94.92% vs 82.46% ;χ2 = 6. 3096,6. 6646,4.5365,5. 6991,4. 5225,respectively; P 〈 0.051. Overall life quality score of experimental group （6.52 ± 0.53 ） was higher than that in control group （4.36 ± 0.78 ）, and the difference was statistically significant （t = 17.498,P 〈 0.05 ）. All the items of QL-Index showed a significantly statistieal difference between two groups （ P 〈 0. 05 ）. Conclusions Applying PDCA circulation can enhance the compliance and quality of life of patients undergoing liver transplantation and improve their satisfaction degree to the nursing work.
