摘要
目的探讨急性化脓性中耳炎的细菌学特征与观察洛美沙星滴耳液的治疗效果。方法收集184例(227耳)急性化脓性中耳炎的脓液标本,进行细菌培养,分别给予采用盐酸洛美沙星滴耳液治疗(观察组)与氧氟沙星滴耳液治疗(对照组),比较两组的细菌学疗效。结果 227耳培养阳性183耳,阳性率80.6%;革兰阳性菌135株,占73.8%,革兰阴性菌48株,占26.2%,分布最多的3种病原菌为金黄色葡萄球菌、流感嗜血菌和铜绿假单胞菌,分别占36.6%、15.3%和13.1%;两组细菌培养阳性率与分布比较,差异无统计学意义;观察组与对照组细菌清除率分别为86.9%和81.0%,差异无统计学意义。结论急性化脓性中耳炎感染以革兰阳性菌为主,洛美沙星滴耳液细菌清除率高,是治疗急性化脓性中耳炎非常有效的药物。
OBJECTIVE To explore the bacterial distribution of acute suppurative otitis medium and investigate the effect of lomefloxacin hydrochloride ear drops on treatment of the disease.METHODS The pus specimens collected from 184 cases（227 ears） of patients with acute suppurative otitis medium were cultured.They were respectively treated with lomefloxacin hydrochloride ear drops（the observation group） or ofloxacin ear drops（the control group）,and bacteriological efficacy between the two groups was compared.RESULTS Totally 183 of 227 ears were cultured positive with the positive rate of 80.6%.There were 135（73.8%） strains of gram-positive bacteria and 48（26.2%） strains of gram-negative bacteria,and the Staphylococcus aureus,Haemophilus influenzae and Pseudomonas aeruginosa were three most common species of pathogens,accounting for 36.6%,15.3% and 13.1%,respectively.The difference in the positive rate of bacterial culture between the two groups was not statistically significant.The bacterial clearance rate was 86.9% in the observation group and 81.0% in the control group,the difference was statistically significant（P0.05）.CONCLUSION The gram-positive bacteria are dominant in the pathogens causing the acute suppurative otitis medium,the bacterial clearance rate of lomefloxacin hydrochloride ear drops is high,and it is an effective drug in the treatment of acute suppurative otitis medium.
