摘要
目的探讨不同分娩方式对产妇初乳分泌及初乳中免疫球蛋白A(sIgA)含量的影响,寻找降低剖宫产率的预防对策。方法选择自然分娩(Ⅰ组)、剖宫产(Ⅱ组)及剖宫产术后使用吗啡(Ⅲ组)的初产妇各40例,记录分娩后首次产生初乳的时间,测定泌乳量和测定首次产生初乳及初乳产生后24、48 h初乳中sIgA含量。结果产生首次初乳的时间顺序为Ⅰ<Ⅲ<Ⅱ(P均<0.01);初乳中sIgA值:首次初乳Ⅰ>Ⅲ>Ⅱ(P均<0.01);初乳产生后24 hⅡ<Ⅰ及Ⅲ(P均<0.05);初乳产生后48 h三组含量差异无统计学意义(P>0.05)。结论自然分娩有利于产妇缩短初乳分泌时间,增加分泌乳量及初乳中sIgA含量。因此,提倡自然阴道分娩。
Objective To investigate the imipact of cesarean section on maternal colostrum and colostrnm secretory im- munoglobulin A ( sIgA ). Methods The 120 lying-in women were divided into natual delivery group(group I), sesarean sec- tion group (Group II)and sesarean section with analgesia group with morphine(group II1) each consisted of 40 cses.The colos- trum lactation time and quantity after delivery were recorded,and the concentrations of sIgA in colostrum were determined 24 hours, 48 hours after production. Results The colostrum lactation time were in the order of group I 〈 group III〈group 11, showing significant difference among the three groups( P 〈 0.01 );The sIgA concentrations of first colostrum were in the order of group I 〉group Iil 〉group II ( P 〈 0.01 ); The amount of solostrum lactated in group I, II and III aaccounted for 37.50% (15/ 40),7.50% (3/40)and 25.00% (10/40)of the total amount needed, showing significant differences among the three groups(P 〈 0.05) The amount of milk lactated 24 hours after the lactation of colostrum were not significantly different among the three groups ( P〉0.05 ). Conclusion Vaginal delivery is contribusive to early lactation of colostrum, increase the lqctation of co- lostrum amount and enhancement of sIgA concentration.
