摘要
目的探讨腹腔镜输卵管妊娠保守性手术中垂体后叶素的应用价值。方法选择2010年1月～2011年12月在我院妇科确诊为未破裂型输卵管妊娠(除输卵管伞部妊娠之外)、有生育要求、具有保留输卵管条件的100例患者,行腹腔镜输卵管妊娠切开取胚保守性手术。随机将其分为两组,应用垂体后叶素为观察组(50例),未使用垂体后叶素为对照组(50例)。比较两组手术时间、术中出血量、输卵管保留成功率、输卵管术后通畅率及持续性异位妊娠发生率情况。结果观察组手术时间、术中出血量明显少于对照组,差异有高度统计学意义(P<0.01);输卵管保留成功率明显高于对照组,差异有高度统计学意义(P<0.01);术后输卵管通畅率亦高于对照组,差异有高度统计学意义(P<0.01)。术后持续性异位妊娠发生率比较差异无统计学意义(P>0.05)。结论输卵管妊娠保守性手术中,使用垂体后叶素能减少手术中出血量,缩短手术时间,提高输卵管保留成功率及输卵管术后通畅率,手术安全可靠,副作用少,值得推广。
Objective To evaluate the efficacy of Pituitrin administration in laparoscopic conservative surgery of tubal pregnancy.Methods 100 patients who were diagnosed as unruptured tubal pregnancy（except the fallopian tube pregnancy）,wanted to preserve fertility and seemed possible for them to reserve the fallopian tube in our hospital from January 2010 to December 2011 were selected and treated with aparoscopic conservative surgery.All the patients were divided into observation group（n = 50） and control group（n = 50） randomly,patients in observation group were treated with Pituitrin during the operation,patients in control group were treated without Pituitrin.The surgical time,intraoperative bleeding,success tube reserved rate,postoperative fallopian tubal patency rate and persistent ectopic pregnancy rate were compared between the two groups.Results Surgical time,intraoperative bleeding in the observation group were significantly less than those in the control group,the differences were all statistically significant（all P〈0.01）;success rate of tubal reserved in the observation group was also higher than that in the control group,the difference was statistically significant（P〈0.01）;postoperative fallopian tubal patency rate in the observation group was higher than that in the control group,the difference was statistically significant（P〈0.05）.The difference of postoperative persistent ectopic pregnancy incidence between the two groups was not statistically significant（P〈0.05）.Conclusion Administration of Pituitary in laparoscopic conservative surgery of tubal pregnancy can reduce the amount of bleeding during surgery,shorten the surgical time,improve the success rate of fallopian tube conservation and tubal patency after operation with less side effect,it is worthy of popularization.
