弹性髓内钉治疗儿童长骨干骨折疗效观察 被引量:15

摘要 目的探讨应用闭合复位弹性髓内钉治疗儿童长骨干骨折的临床疗效。方法选择2007年7月~2011年6月我院采用闭合复位弹性髓内钉内固定术治疗儿童四肢长骨干闭合骨折患儿134例。所有病例均记录切口长度、手术时间、骨折X线愈合时间、术后并发症及关节功能情况。结果术后切口均Ⅰ期愈合,无切口感染。手术切口长度平均2.15 cm,平均手术时间为50 min。134例均获得随访,平均随访时间1.5年(1~3年),无骨折再移位、骨折不愈合及延迟愈合,骨折愈合时间平均为7周,术后平均6个月取出内固定。有2例患儿出现钉尾带来的软组织激惹及滑囊形成,取出内固定后消失。末次随访时患侧肢体长度较健侧平均过度生长0.75 cm,骨折部位上下两个关节功能与健侧相比无明显差异,5例患儿前臂旋转功能较健侧轻度受限,未见成角畸形。结论对完全移位的儿童四肢长骨干闭合骨折,闭合复位弹性钉手术操作简单、创伤小、愈合快、预后好,应作为首选的治疗方法。 Objective To evaluate the curative effect of application of closed reduction titanium elastic nails in the treatment of children's long bone shaft fracture.Methods A total of 134 cases of children with long bone shaft closed fracture treated with closed reduction titanium elastic nails in our hospital from July 2007 to June 2011 were selected.All the cases were recorded the surgical incision lengths,mean surgery time,fracture X-ray cure time,post-operation complication and the joint function.Results Postoperative incisions were all stage Ⅰ healing,with no incision infection.The average surgical incision length was 2.15 cm and the mean surgery time was 50 min.All 134 patients were followed up for 1-3 years,with an average of 1.5 years.There was no fracture redisplacement,nonunion or delayed union occurred.The average healing time was 7 weeks,then the internal fixation was removed after 6 months.Nail tail irritation and synovial sac formation happened in 2 patients,which disappeared after removal of implants.In the last follow-up,the injury limbs had overgrown 0.75 cm.Compared with contralateral,the function of two joints up and down the fracture sites had no significant difference.Compared with contralateral,5 cases forearm rotation was mild limited,with no angular deformity.Conclusion For completely displaced children's long bone shaft closed fracture,closed reduction titanium elastic nails has advantages of simple operation,minimal invasion,faster healing and better prognosis.It should be used as a preferred method for treatment.
