摘要 目的通过比较溶血性贫血(HA)患者与正常人的血红蛋白浓度,探讨平均红细胞血红蛋白浓度(MCHC)对溶血性贫血诊断的临床意义。方法统计HA、小细胞HA(先天遗传小细胞或合并IDA)、缺铁性贫血(IDA)、巨幼细胞性贫血(MA)各50例患者外周血象平均红细胞血红蛋白量(MCH)、MCHC、平均红细胞体积(MCV)数据,以健康人群50名为对照组,比较各实验组与对照组的MCH、MCHC、MCV均值。结果外周血象MCHC比较显示HA患者MCHC明显比MA、IDA及对照组要高,差异有统计学意义(P<0.01);小细胞HA患者和MA患者外周血象MCHC与对照组差异无统计学意义(P>0.05),而小细胞HA患者MCV明显比对照组低(P<0.01),MA患者MCV则明显高于对照组(P﹤0.01);外周血象MCHC最低的IDA患者与对照组及其他实验组差异有统计学意义(P<0.01)。结论 MCHC可作为早期溶血性贫血的诊断指标。 Objective To compare the hemoglobin concentration for patients with hemolytic anemia and the normal" and to explore the clinical significance of the mean corpuscular hemoglobin concentration (MCHC) for hemolytic ane- mia. Methods In each 50 patients (experimental group) of HA, small cell HA (of congenital small cell or merge IDA), iron deficiency anemia (IDA), megaloblastic anemia (MA), the peripheral blood mean corpuscular hemoglobin amount (MCH), MCHC mean corpuscular volume (MCV) data were taken for statistics. 50 health people were selected as con- trol group, the MCH, MCHC and MCV mean level between the two groups were taken for comparison. Results The peripheral blood MCHC for HA patients was significantly higher than MA, IDA and control group (P 〈 0.01). The IDA peripheral blood of patients like MCHC was significantly lower than the control group and the experimental group (P 〈 0.01). MA in peripheral blood of patients like MCHC compared with the control group, the difference was not statisti- cally significant (P 〉 0.05). MCV was significantly higher than that in the control group (P 〈 0.01). The small cell HA peripheral blood the MCHC compared with the control group, the difference was not statistically significant (P 〉 0.05), while MCV was obvious lower than the control group (P 〈 0.01). Conclusion MCHC can be used as an early diagnos- tic indicator of hemolytic anemia.
