摘要
目的探讨腹部闭合性损伤患者凝血功能检测的临床意义。方法将89例腹部闭合性损伤患者根据腹腔出血量分为41例小于2000mL组和48例大于或等于2000mL组,与50例健康体检者(对照组)进行血浆凝血功能比较。89例患者经治疗后分为痊愈组86例与死亡组3例。应用全自动血凝仪、全自动血细胞计数仪分别检测血浆凝血酶原时间(PT)、凝血酶原时间国际标准化指数(INR)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、血小板(PLT)。结果出血量小于2000mL组、大于或等于2000mL组与对照组比较,差异有统计学意义(P<0.05);出血量大于或等于2000mL组与小于2000mL组比较,PT、INR、APTT、TT均明显延长,PLT、FIB明显降低(P<0.05)。痊愈组与死亡组凝血功能比较,差异有统计学意义(P<0.05)。结论腹部闭合性损伤患者凝血功能检测在判断患者治疗及预后方面有重要临床意义,同时亦为临床纠正凝血功能严重紊乱,改善预后提供诊断依据。
Objective To explore clinical significance on coagulant function detection of blunt abdominal trauma. Methods According to the amount of bleeding, 89 cases of blunt abdominal trauma patients were divided into less than 2 000 mL group（41cases） and at least 2 000 mL group（48cases） ,compared with 50 healthy controls（control group） of plasma coagulant function. 89 cases after treatment were divided for 86 cases of recovery group and death group in 3 cases. Automated blood coagulation analyzer, automatic blood cell counter respectively were used to detect prothrombin time（PT）, prothrombin time,international normalized ratio （INR） ,activated partial thromboplastin time（APTT） ,thrombin time（TT） ,fibrinogen（FIB） and platelet（PLT）. Results Less than 2 000 mL group and at least 2 000 mL group compared with control group,the difference was statistically significant（P〈0.05）. At least 2 000 mL group compared with less than 2 000 mL group,PT,INR,APTT coagulation function were significantly prolonged, TT, PLT, FIB were significantly decreased（P〈0.05）. There was statistical difference of coagulation function between cure group and death group（P〈0.05）. Conclusion Coagulant function detection of blunt abdominal trauma which have important clinical significance in Judging patients treatment and prognosis.
