摘要
目的探讨持续低负压吸引应用于四肢软组织损伤清创术后的护理效果。方法方便性抽样选取上海市第六人民医院骨科行四肢软组织缺损清创负压封闭引流(vacuum sealing drainage,VSD)术后3d内的患者129例。2009年1-10月间患者41例采用传统间断式吸引方式即间断吸引组,上肢、下肢损伤患者负压吸引压力分别为200～300kPa、330～450kPa之间;2009年11月至2011年1月间患者88例采用持续低负压吸引方式即持续吸引组,上肢、下肢损伤患者负压吸引压力分别为126～210kPa、250～330kPa之间。比较两组患者在疼痛、管道堵塞及通管次数等方面的差异。结果两组患者初始疼痛评分无差异(P>0.05);但持续吸引组患者术后各时间点疼痛程度均轻于间断吸引组(均P<0.01),且术后72h内管道堵塞、通管次数以及负压调控次数均低于间断吸引组(均P<0.01)。结论对于四肢软组织损伤清创术后患者,在保证有效吸引的同时选取持续低负压吸引,可以提高患者舒适度,降低疼痛程度,减少护理工时,促进伤口的良好生长。
Objective ~ To investigate the nursing effect of continuous low negative pressure suction in the debridement of soft tissue wounds of limbs. Methods A total of 129 patients undergoing VSD debridement of soft tissue wounds of limbs within 3 days in the hospital were enrolled in the study using convenience sampling. Between January and October 2009,41 patients underwent the traditional intermittent suction： suction pressure of 200 - 300 kPa was arranged for patients with upper extremity injuries according to the doctor＇s advice and 250-330 kPa for those with lower extremity injuries. Between November 2009 and January 2011, 88 patients underwent continuous low negative pressure suction （continuous suction group） ： suction pressure of 126-210 kPa was arranged for patients with upper extremity injuries according to the doctor＇s advice and 250-330 kPa for those with lower extremity injuries. Comparisons were conducted on the pains, clogged and dredging tube numbers. Results No significant difference was detected in the initial pain scores of the two groups（P〉0.05）. The comfort levels at each time point were significantly higher in continuous suction group than in discontinuous suction group（P〈0.01）. The scores of pain rating scale, clogged and dredging tube numbers at each time point within 72 h were significantly lower in continuous suction group than in discontinuous suction group（P〈0.01）. Conclusion For patients with soft tissue wounds of limbs, continuous low negative pressure suction based on effective suction can improve patient comfort ,relieve their pains, reduce the working hours of care and complications and promote the growth of the wounds.
