医院感染铜绿假单胞菌的临床分布及多重耐药分析 被引量:8

DISTRIBUTION AND RESISTANCE MECHANISM OF NOSO COMIAL ISOLATES OF MULTI-RESISTANT PSEUDOMONAS AERUGINOSA

摘要 目的了解我院2009年1月—2011年10月铜绿假单胞菌的临床分布及其对常用抗菌药物的敏感性。方法用常规方法将临床送检的各种标本进行分离培养,用VITEK-60细菌鉴定仪进行细菌鉴定和药敏分析。结果检出菌株数及标本来源所占比例依次为:痰78株(74.3%),分泌物16株(15.2%),血液4株(3.8%),其他4株(3.8%),尿液3株(2.9%),共计105株。对哌拉西林/他唑巴坦、头孢哌酮/舒吧坦、头孢他啶敏感性较好,对头孢曲松、头孢噻肟、庆大霉素等耐药率较高。结论多重耐药铜绿假单胞菌比例不断上升,这与细菌可产生多种β内酰胺酶及整合子可介导耐药基因横向播散相关,临床医生务必根据药物敏感试验和本单位耐药监测情况合理选择抗菌药物,以减少多重耐药菌的产生和发展。 Objective To investigate the clinical distribution and drug sensitivity of nosocomial isolates of Pseudomonas aeruginosa,which were isolated during Jan. 2009 - Oct 2011. Methods Kinds of bacteria in clinical species were isolated and cultured with conventional method. VITEK - 60 bacteria identification and sensitivity analyzer was applied. Results 105 strain Pseudomonas aeruginosa were detected . The distribution were as follows:78 strains of sputum samples(74.3), 16 strains of stool samples (15.2%) ,4 strains of blood samples(3.8%) ,4 strains of other samples(3.8%) ,3 strains of urine samples(2.9%). The sensitive antibiotics were Piperacillin/Tazobactam, Cefoperazone/Sulbactam, Ceftazidime, while it was high resistant to Ceftriaxone,Cefotaxime and Gentamicin. Condusion The prevalence of multiresistant P. aeruginosa increases gradually. This is associated with the production of multiple beta - lactamases by P. aeruginosa and transverse spread of resistant genes mediated by Class 1 integron. Clinicians should choose appropriate antibiotic therapy based on bacterial susceptibility to reduce or prevent the emergence and development of multi- resistant strains.
