ICU机械通气患者支气管镜肺泡灌洗疗效分析 被引量:3

摘要 目的:探究支气管镜肺泡灌洗在机械通气患者中的临床治疗优越性。方法:将60例患者分为常规灌洗吸痰组(对照组)和支气管肺泡灌洗组(BAL-观察组)2组,观察2组灌洗前后血气参数PaO2、SaO2%、PaCO2的变化。结果:对照组治疗前后参数分别为PaO2,t=1.79,P>0.05;PaCO2,t=3.51,P<0.05;SaO2%,t=2.29,P<0.05。观察组治疗前后参数为PaO2,t=6.39,P<0.01;PaCO2,t=10.38,P<0.01;SaO2%,t=4.88,P<0.01。观察组与对照组相比较,PaO2,t=2.94,P>0.01;Pa-CO2,t=9.26,P<0.01;SaO2%,t=2.64,P<0.05。观察组和对照组肺泡灌洗后血气参数的SaO2%比灌洗前显著升高(P<0.01),且观察组PaO2和SaO2%比对照组在接受积极治疗后显著升高(P<0.01)。对照组和观察组肺泡灌洗后PaCO2较灌洗前有明显下降(P<0.01),且观察组PaCO2下降较对照组显著(P<0.01)。对照组治疗前后对比中,PaO2虽有回升,但P>0.05,差异无统计学意义。可以看出,经支气管镜肺泡灌洗治疗后,患者的血气均有不同程度好转,治愈率均明显优于对照组,且在治疗中无严重并发症发生,可明显缩短治疗疗程。结论:在ICU机械通气急、危重患者中及时给予支气管镜肺泡灌洗,可较大程度地改善通气,并有效控制肺部感染,减低死亡率,提高预后满意度,缩短治疗疗程。 Objective To explore the clinical treatment superiority of bronchoscopic bronchoalveolar lavage for mechanically ventilated patients. Methods Sixty patients were divided into two groups, of which, thirty ones were in the control group and went through routine lavage & sputum aspiration and the remained ones were in the BAL-experimental group and through bronchoalveolar lavage. The blood gas parameters, such as PaO2, SaO2% and PaCO2, were measured before and after the lavage. Results For the control group before and after lavage, the results were t=1.79 and P〉O.05 for PaO2, t=3.51 and P〈 0.05 for PaCO2, and t=2.29 and P〈0.05 for SaO2%. For the BAL-experimental group, the results were t=6.39 and P〈0.01 for PaO2, t=10.38 and P〈0.01 for PaCO2, and t=4.88 and P〈0.O1 for SAO2%. When the BAL-experimental group was compared with the control group, the results were t=2.94 and P〉0.0l for PaO2, t=9.26 and P〈0.01 for PaCO2, and t=2.64 and P〈0.05 for SAO2%. There were significant increases for SAO2% and decreases for PaCO2 before and after lavage in the both groups, and significant increases for PaO2 and SAO2% and decreases for PaCO2 in the BAL-experimental group compared with those in the control group, with P〈0.01. It's not significant for the increase of PaO2 in the control group, with P〉O.05. Bronchoscopic bronchoalveolar lavage showed its effectiveness for the patients, with no serious complications and shortened treatment course. Conclusion Timely bronchoscopic bronchoalveolar lavage is of great benefit for the patients with mechanical ventilation from the aspects of respiration, controlled pulmonary infection, decreased mortality, improved prognosis and shortened treatment course.
