四肢皮肤撕脱伤37例治疗体会 被引量:3

摘要 目的探讨皮肤撕脱伤的临床特点及治疗方法。方法 2005年1月至2011年6月我科收治37例四肢皮肤撕脱伤患者,根据撕脱皮肤血运情况选择原位缝合法5例、撕脱皮片回植法15例、撕脱皮片回植并持续封闭负压引流(VSD)技术17例,观察皮瓣及皮片成活情况,创面远期恢复、肢体外观及功能恢复情况。结果原位缝合法优良率为60%,撕脱皮片回植法优良率为80%,撕脱皮片回植并VSD技术优良率为88%,平均获得12个月随访,采用上述方法修复四肢皮肤撕脱伤收到较好效果。回植皮肤颜色接近周围皮肤,耐磨性好,肢体功能恢复满意。结论根据撕脱皮肤血运情况制定合适治疗方案是治疗皮肤撕脱伤的关键。 Objective To investigate the clinical characteristics and treatment method of limb skin avulsion. Methods Thirty-seven patients of limb skin avulsions were treated in the Orthopaedic Center from January 2005 to June 2011. According to avulsed skin blood supply situation, 5 patients were treated by in situ seam, 15 patients were treated by tore peeling back to plant, and 17 were treated by tore peeling back to plant and continued vacuum sealing drainage (VSD). The skin flap and peeling situation, wounds, body appearance and long-term recovery function were observed. Results The excellent and good rate was 60% for in situ seam, 80% for tore peeling back to plant, and 88% for tore peeling back to plant and continued vacuum sealing drainage (VSD). During the 12 months' follow-up, the appearance of the skin in all the patients was satisfactory and limb function was good. Conclusion It is the key for the treatment of limb skin avulsion to develop appropriate treatment according to the avulsed skin blood supply conditions.
