尿毒症透析前后免疫及炎性状态的变化规律研究 被引量:3

摘要 目的:探讨尿毒症血液透析患者透析前后免疫及炎性状态的变化规律。方法:选取2010年8月~2012年3月于本院进行血液透析的41例尿毒症患者为观察组,同期41例健康人员为对照组,将观察组透析前,透析后2、4、8周同对照组的免疫球蛋白及其补体C3、C4、红细胞免疫指标和血清白细胞介素-6(IL-6)、C反应蛋白(CRP)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)进行比较。结果:观察组透析前至透析后2、4、8周免疫球蛋白及其补体C3、C4、红细胞免疫促进因子(RFER)、红细胞C3b受体(RBC-C3bR)持续升高,血清IL-6、CRP、IL-8、TNF-α、红细胞免疫抑制率(RFIR)及红细胞免疫复合花环(RBC-ICR)持续降低,差异有统计学意义(P均<0.05)。结论:血液透析可明显改善尿毒症患者的免疫及炎性状态。 Objective= To study the variation of immune state and inflammatory state of patients with uremia before and after hemodialysis. Methods. A total of 41 patients with uremia who were treated with hemodialysis in our hospital from August 2010 to March 2012 were selected as observation group,and 41 healthy personnel at the same time were selected as observation group. Immunoglobulin and its comple- ment C3, C4, erythrocyte immune indexes and serum IL-6, CRP, IL-8, TNF-α of observation group before and at the second, forth and eighth week after hemodialysis were measured and compared with control group. Results= The immunoglobulin and its complement C3, C4, RFER and RBC-C3bR of observation group showed continuously significant increase before and after hemodialysis, serum IL-6, CRP, IL-8, TNF-α,RFIR and RBC-ICR showed continuously significant decrease (all P〈0.05). Oonclusions. Hemodialysis can significantly improve the immune state and inflammatory state of patients with uremia.
