摘要
目的了解2010年牡丹江医学院红旗医院临床分离的大肠埃希菌及肺炎克雷伯菌的耐药性变化,为临床合理用药提供依据。方法取自2010年医院各临床科室送检的全部标本,对其进行菌种鉴定及药物敏感性分析,对仪器提示为产ESBLs的菌种,进行确证试验。结果 2010年医院大肠埃希菌检出80株,其中产ESBLs为19株,阳性率为23.8%;肺炎克雷伯菌检出89株,其中产ESBLs为19株,阳性率为21.3%;产ESBLs大肠埃希菌和肺炎克雷伯菌对亚胺培南、美罗培南的耐药率为0,对大多数抗菌药物的耐药率明显高于非产ESBLs菌株(P<0.05)。结论产ESBLs菌株在临床的检出率越来越高,耐药率明显高于非产ESBLs株,且呈多药耐药性,给临床诊治带来极大困难;加强产ESBLs菌株的监测及耐药性分析,指导临床合理应用抗菌药物,对控制产ESBLs菌株的播散和流行具有重要意义。
OBJECTIVE To analyze the clinical isolation and drug resistance of Escherichia coli and Klebsiella pneumonia that were isolated from Hongqi Hospital in 2010 so as to provide basis of the reasonable clinical medication. METHODS The identification and antibiotic susceptibility testing of the bacteria were determined by microorganism analyzer in all the specimens from clinical departments. ESBLs-producing strains were confirmed with double disc method. RESULTS A total of 80 strains E. coli were isolated in 2010, among which there were 19 strains of ESBLs-produeing E. coli with the positive rate of 23. 8%, and 89 strains of K. pneumonia were isolated,among which there were 19 strains of ESBLs-producing K. pneumoniae with the positive rate of 21.3%. The drug resistance rates of the ESBLs-producing E. coli and K. pneumoniae to imipenem and meropenem were 0%. The drug resistance rate of the ESBLs-producing strains to most antibiotics was significantly higher than that of the non-ESBLs-producing strains. CONCLUSION The detection rate of the clinically isolated ESBLs-producing strains is increasiny,the drug resistance rate of which is higher than that of the non-ESBLs-produeing strains. The isolated strains are multidrug-resistant, which bring great difficulties to the clinical treatment. It is necessary to strengthen the monitoring of drug resistance of the ESBLs-producing strains and guide the reasonable clinical medication so as to prevention the prevalence and spread of the ESBLs-producing strains.
