摘要
目的 探讨胰十二指肠切除术术前可切除性的评估和手术方法的改进对切除率、并发症发生率、手术死亡率和生存率的影响。方法 阐述壶腹周围癌CT的鉴别诊断与术前手术可切除性的评估的方法 ,以及对壶腹周围癌 ,尤其是胰头癌行胰十二指肠切除术的手术方法的改进。结果 138例壶腹周围癌中 ,行胰十二指肠切除术 98例 ,切除率 71% ,并发症发生率 16 7% ,手术死亡率6 5 %。其中胰头癌 112例 ,行胰十二指肠切除术 72例 ,切除率 6 4% ;胰头癌术后 1、3、5年生存率分别为 70 %、2 5 %和 12 %。结论 术前准确评估手术可切除性 ,不断改进手术方法 ,有利于壶腹周围癌 ,尤其是胰头癌的切除率和生存率的提高 。
Objective To discuss the preoperative prediction of resectability and the improvement of surgical techniques of pancreaticodudenectomy for periampullary carcinoma and their effects on the resection rate,operative morbidity,operative mortality and survival rate.Methods The differential diagnosis and the method of preoperative evaluation of resectability by computed tomography,and the improvement of surgical techniques of pancreaticodudenectomy for periampullary carcinoma,especially carcinoma of the head of the pancreas were explicated.Results Of 138 cases of periampullary carcinoma,pancreaticodudenectomy was performed in 98 cases.The resection rate was 70%,operative morbidity was 16 7% and operative mortality was 6 5%.Of the 112 cases of carcinoma of the pancreatic head,pancreaticodudenectomy was conducted in 72 cases.The resection rate was 64% and the postoperative 1 ,3 ,5 year survival rate for carcinoma of the pancreatic head were 70%,25%,and 12% respectively.Conclusions Accurate preoperative prediction of resectability and continuous improvement of surgical techniques are of great importance in improving the resection rate and survival rate,and in reducing the operative morbidity and mortality of periampullary carcinoma,especially carcinoma of the head of the pancreas.
