微电极导向苍白球腹后部毁损术治疗帕金森病

摘要 目的 观察微电极导向苍白球腹后部毁损术治疗帕金森病的临床疗效。 方法 应用微电极导向苍白球腹后部毁损术治疗 2 4例帕金森病患者 ,分析其术后临床症状的改善情况 ;平均随访时间 4.2个月。 结果 术中靶点调整率 82 .5 % ,手术有效率 10 0 % ;不同症状的改善率 (% )分别为 :僵直 91.0 ,运动迟缓 86 .7,震颤 83.3,步态 73.4,平衡 6 8.5 ,“关”状态 76 .0。左旋多巴引起的并发症基本消除 ,无严重及永久并发症。 结论 微电极导向苍白球腹后部毁损术治疗帕金森病 ,具有比传统的丘脑毁损术更加全面的治疗效果 ,尤其对运动迟缓和左旋多巴诱导的并发症更具治疗优势 ,副作用较少。微电极导向确保靶点的精确定位是手术成功的关键 。 Objective\ To observe the outcome of posteroventral pallidotomy(PVP) for Parkinson's disease(PD).\ Methods\ The date from 24 patients with PD receiving microelectrode guided PVP were retrospectively analyzed with unified Parkinson's disease rating scale(UPDRS).\ The median follow\|up period was 4 2 months(ranged from 2 5 to 7 moths).\ Results\ The change rate of intraoperative target localization was 82 5%.\ The outcome was excellent.\ Several typical symptoms were improved: rigidity 91%, bradykinesia 86 7%, tremor 83 3%, gait 73 4%, balance 68 5% and “off” state 76% \ Preoperative and postoperative Motor UPDRS score, both of “on” and “off” state were significantly improved.\ Drug\|induced motor fluctuations and dyskinesia were distinctly alleviated.\ No patient showed severe and permanent complications.\ Conclusion\ Microelectrode guided PVP for PD is superior to classical thalamotomy in the improvement of parkinsonian symptoms, especially for bradykinesia\|akinesia, and has less side effects on brain functions.\ Microelectrode recording is essential to surgical results and avoid of severe complications.\ There will be subject to follow\|up for long term outcome.\;
