摘要
目的 总结干下型室间隔缺损 (SIVSD)外科治疗的经验。方法 全部采用中低温体外循环下经肺动脉补片修补。 2例合并主动脉右冠窦瘤及 1例合并动脉导管未闭同期手术 ,2例伴主动脉瓣脱垂及 2例合并主动脉瓣轻度关闭不全仅作室缺修补。结果 全组无死亡。复查无残余分流。15例胸骨左缘 2～ 3肋间残留 1～ 2 6SM ,3～ 6月后杂音减弱或消失。结论 适时手术 ,采用补片修补 ;
Objective To summarize the experience of surgical treatment of subarterial ventricular septal defect(SIVSD).Methods All cases were repaired with a patch through pulmonary artery incision under middle hypothermia extracorporeal circucation with 1 cases complicated with aneurysm of right coronary sinus and 1 cases with patent arterial duct under concomitant surgery,and 2 cases with aortic valve prolapse and 2 cases with complication of mild aortic incompetence only underwent VSD repair.Results There were no operative deaths and no residual intracardiac shunt in reexamination.1-2/6 degrees of systolic murmur(SM) were heard at left margin of sternum and 2-3 intercostal space in 15 patients and 3-6 months later,the murmur weakened or disappeared.Conclusion Appropriate timing of operation,using patch repair and correct judgement and management of pathologic changes in aortic valve were the keys to improving the operative effects on the patients with subarterial ventricalar defect.
