摘要
目的:分析静脉药物配置中心(Pharmacy Int ravenous Admi xt ure Servi,cPeIVAS)的不合理处方,保证患者用药安全。方法:临床药师审查静脉药物配置中心医嘱,根据临床药理学知识及文献分析不合理情况。结果:共审查医嘱2235张,发现不合理72张,主要包括配伍禁忌、溶媒选择错误,药物浓度不合适,给药间隔不合理、医嘱录入错误等。通过药师干预,不合理医嘱出现率由6.7%下降到1.63%。结论:PIVAS的开展,可减少不合理医嘱,指导临床合理用药。
Objective To analyze of the unreasonable prescriptions in pharmacy intravenous admixture services and to ensure patients medication safety. Methods Clinical pharmacists examined the prescriptions in PIVAS and analyzed the unreasonable prescriptions ac- cording to pharmacological knowledge and literatures. Results A totie of 2235 prescriptions was examined, and 72 pieces of unreason- able prescriptions were found,invorving problems in incompatibility, improper choice of infusion carriers,unsuitable concentration,improper interval of drug administration and erroneous advising. With the intervention of crinical pharmacist, the rate of irrational prescriptions were reduceds from 6.7% to 1.63%. Conclusion PIVAS can decrease the irrational administration and guide clinical rational use of drug.
