摘要
目的对单用甲钴胺和前列地尔联合甲钴胺治疗糖尿病神经周围病变的疗效和不良反应进行评价。方法对PubMed、Cochrane Library、万方、维普(VIP)、Ovid、CNKI、中国生物医学文献数据库(英文简称CBM)、西文生物医学期刊文献数据库等数据库2011年2月之前的关于单用甲钴胺和前列地尔联合甲钴胺治疗糖尿病神经周围病变的疗效和安全性的随机对照试验(RCT)的文献进行计算机检索。两位研究者依据Cochrane系统评价手册5.0的文献质量评价方法,对检索文献进行独立筛选,并交叉核对,将质量好的文章纳入研究,进行Meta分析采用RevMan5024软件,该软件由Cochrane协作网提供。结果共有34篇文献纳入研究,合计1892例患者。通过Meta分析,发现:前列地尔联合甲钴胺与单用甲钴胺治相比,联合用药组治愈率高于单用组,前列地尔联合甲钴胺治疗后比单用甲钴胺神经传导速度均明显提高,住院时间短(MD=-4.0,95%CI-6.84～-1.16,P=0.006),而不良反应发生率经比较,差异无统计学意义。结论前列地尔联合甲钴胺治疗糖尿病神经周围病变与单用甲钴胺相比,是一种安全有效的保守治疗措施。
Objective To single with mecobalamin and effects of alprostadil and Mecobalamin on diabetic peripheral neuropathy Nerve efficacy and adverse reaction were evaluated.Methods PubMed,Library,Cochrane million,VIP（VIP）,Ovid,CNKI,Chinese biomedical literature database（English abbreviation CBM）,Western biomedical periodicals database database from 2011 until February on the single with mecobalamin and alprostadil and Mecobalamin for diabetic peripheral nerve lesions of the efficacy and safety of randomized controlled trials（RCT） of computer retrieval.The two researchers on the basis of Cochrane System Evaluation Handbook 5 literature quality evaluation method of literature retrieval,independent screening,and cross check,will be good quality articles included in the study,Meta analysis by using RevMan50 24 software,the software provided by Cochrane collaboration.Results A total of 34 articles were included in the study,a total of 1892 patients.Through the Meta analysis,we find：effects of alprostadil and Mecobalamin in treating compared with single,combined therapy group cure rate is higher than that in single group,the effect of alprostadil combined with Mecobalamin after treatment than the single with Mecobalamin on nerve conduction velocity were significantly improved,shorter hospitalization time（MD= 4,95%CI-6.84~ 1.16,P= 0.006）,but the bad reaction rates of the differences were not statistically significant.Conclusion Alprostadil and Mecobalamin on diabetic peripheral neuropathy Nerve with Mecobalamin compared,is a safe and effective conservative treatment measures
