摘要
目的对血浆置换术应用于重症肝病进行治疗性研究。方法收集2009～2011年在我科进行血浆置换治疗的重症肝病患者共25例,监测治疗前后生化指标的变化情况及转归情况。结果治疗前后患者的临床症状及生化检查均有不同程度的改善,治疗前后总胆红素（TBIL）分别为（542.50±163.96）μmol/L和（295.50±150.34）μmol/L,治疗前后血清白蛋白（ALB）分别为（32.06±4.35）g/L和（34.54±3.97）g/L,治疗前后凝血酶原活动度（PTA）分别为（35.04±15.43）%和（58.97±23.46）%。治疗后患者血清TBIL下降显著（P〈0.01）,ALB、PTA上升明显（P〈0.05）。25例患者作血浆置换治疗68次,有效出院者16例,无效出院者9例,治疗次数2~4次有效率为60%。结论大剂量血浆置换技术应用于严重肝病患者的治疗疗效明显,对血浆置换的方法和方式进行研究,有利于减少血浆的使用量。
Objective To study on plasma exchange in the treatment of the patients with severe liver disease.Methods 25 cases of patients with severe liver disease in our department who received plasma exchange from 2009 to 2011,and the biochemical changes and outcome of the patients before and after the treatment were monitored.Results Clinical symptoms and biochemical test of all patients had different degrees of improvement after the treatment.Before and after plasma exchange,the levels of serum total bilirubins（TBIL） were（542.50±163.96）μmol/L and（295.50±150.34）μmol/L,serum Albumin（ALB） were（32.06±4.35）g/L and（34.54±3.97）g/L,prothrombin activities（PTA） were（35.04±15.43）% and（58.97±23.46）% respectively.The patient′s serum TBIL before treatment was significantly lower than that after treatment（P 0.01）,while the serum albumin,PTA of the patients after treatment significantly increased than that before treatment（P 0.05）.25 patients received 68 times of plasma exchange,of which 16 cases was effective while 9 cases was ineffective.The effective rate was 60% in patients who received 2-4 times of plasma exchange.Conclusion High dose of plasma exchange is one of the effective methods in the treatment of the patients with severe liver disease,to study ways and means of plasma exchange are benefit to the reduction of plasma use.
