摘要
目的涎腺腺样囊性癌(salivary adenoid cystic carcinoma,SACC)是常见的涎腺恶性肿瘤,血管内皮生长因子(vas-cular endothelial growth factor,VEGF)在不同临床病理特征的SACC中表达情况不尽相同。文中系统评价有关SACC组织中VEGF表达及其临床病理特征之间的关系。方法计算机检索Cochrane Library、PubMedline、中国生物医学文献数据库(ChinaBioMedical Bibliographic Database,CBMdisc)、中国学术期刊全文数据库(Chinese National Knowledge Infrastructure,CNKI)、中文科技期刊全文数据库(VIP Database for Chinese Technical Periodicals,VIP)、万方数据库,从建库至2012年1月31日,并辅以文献追溯的方法,收集国内外公开发表的所有相关病例对照研究。按纳入排除标准筛选文献并评价纳入研究的质量。应用RevMan 5.1软件进行Meta分析。结果共纳入9个病例对照研究,其中腺样囊性癌(adenoid cystic carcinoma,ACC)381例,正常涎腺对照81例。Meta分析结果显示,VEGF在SACC组与正常涎腺对照组的表达差异有统计学意义[OR=13.33,95%CI(6.98,25.45)];VEGF在ACC淋巴结转移组与非淋巴结转移组的表达差异有统计学意义[OR=3.97,95%CI(1.60,9.87)];VEGF在ACC临床Ⅰ~Ⅱ期组与临床Ⅲ~Ⅳ期组的表达差异有统计学意义[OR=0.37,95%CI(0.20,0.67)];VEGF在ACC腺样/管状型组与实性型组的表达差异有统计学意义[OR=0.15,95%CI(0.07,0.32)];VEGF的表达水平与ACC有无神经侵袭或远处转移均无统计学意义。结论 VEGF高表达与SACC淋巴结转移、临床分期、病理分型等临床病理特征显著相关,但与神经侵袭、远处转移无关。受纳入文献质量的影响,以上结论需要高质量的病例对照研究进一步证实。
Objective Salivary adenoid cystic carcinoma (SACC) is a common malignancy in salivary glands, and the ex- pression of the vascular endothelial growth factor (VEGF) varies SACC with different clinicopathologic characteristics. The study aims to investigate the expression of VEGF in SACC and explore the relationship between the VEGF expression and the clinicopathologiccharacteristics of SACC. Methods Published literature relevant to the expression of VEGF in SACC and its relationship with the clinicopathologic characteristics of SACC was identified by searching the Cochrane Library, PubMed, CBMdisc, CNKI, VIP and WAN- FANG Database, and a manual search of other journals was also con- ducted for relevant case-control trials. The quality of the includedstudies was assessed, and Cochrane Collaborationg software RevMan 5.1 was used to test the heterogeneity, overall effect and publica- tion bias of the studies. Results A total of 9 case-control studies were included, involving 381 cases of SACC and 81 normal sali- vary gland controls. Meta-analysis showed statistically significant differences in the VEGF expression between the SACC cases and nor- mal controls ( OR = 13.33, 95 % CI [ 6.98 - 25.45 ] ), lymph node metastasis and non-lymph node metastasis ( OR = 3.97, 95% CI [ 1.60 - 9.87 ] ), clinical stages Ⅰ - Ⅱ and Ⅲ - Ⅳ ( OR = 0. 37, 95% CI [ 0.20 - 0.67 ] ), and the adenoid/cystic type and solid type with OR ( OR =0.15, 95% CI [0.07 -0.32] ). There was between The expression level of VEGF exhibited no significant corre- lation with perineural invasion or distant metastasis of SACC. Concluslon Higher VEGF expression was correlated with the lymph node metastasis, clinical stages and pathological types of SACC, but not with its perineural invasion and distant metastasis. However, our conclusion is to be further supported by more high quality case-control studies.
