摘要
目的探讨肺大细胞癌的临床特点和影响生存率的因素。方法回顾性分析2004~2008年于该科诊治的24例肺大细胞癌患者的临床资料,并分析影响生存率的因素。结果肺大细胞癌男女比7∶1,平均年龄57.75岁,平均直径5.63 cm,既往吸烟占95.83%,肿瘤位于上肺占75.00%,大细胞神经内分泌癌占37.50%。随访期间死亡率66.67%。根治性治疗、除大细胞神经内分泌癌之外的大细胞肺癌、淋巴结阴性、Ⅰ~Ⅱ期患者的3年生存率分别显著高于姑息性治疗、大细胞神经内分泌癌、淋巴结阳性和Ⅲ~Ⅳ期患者(P=0.000、P=0.026、P=0.005和P=0.005)。结论肺大细胞癌多见于中老年男性吸烟患者,肿瘤直径较大,好发于上肺,部分肿瘤伴神经内分泌分化,恶性程度高。姑息性治疗、大细胞神经内分泌癌、淋巴结阳性、Ⅲ~Ⅳ期的患者预后差。
【Objective】 To investigate clinical features and factors which impacted survival rates of large cell lung cancer.【Methods】 The clinical data of 24 cases of large cell lung cancer diagnosed and treated in our department from 2004 to 2008 were retrospectively analyzed,and factors which impacted survival rates were also analyzed.【Results】 The ratio of male to female in large cell lung cancer was 7:1,the average age was 57.75 years and the average diameter was 5.63 cm.95.83% of all patients had a smoking history,75.00% were located in the upper lobe and 37.50% were large cell neuroendocrine carcinoma.The death rate was 66.67% in the follow up period.The 3-year survival rates in patients with radical treatment,other large cell lung cancer besides large cell neuroendocrine carcinoma,negative lymph nodes and stage I~II were significantly higher than those with palliative treatment,large cell neuroendocrine carcinoma,positive lymph nodes and stage III~IV respectively(P =0.000,P =0.026,P =0.005 and P =0.005).【Conclusion】 Large cell lung cancer more frequently occurred in middle-aged and elderly male patients with smoking history and the diameters of the tumors were often large.The tumors were commonly located in the upper lobe and some tumors were associated with neuroendocrine differentiation.The tumors were always highly malignant.The prognosis were poor in patients with palliative treatment,large cell neuroendocrine carcinoma,positive lymph nodes and stage III~IV.
